不同途径骨髓间充质干细胞移植后脊髓损伤大鼠运动功能比较的Meta分析 被引量:3

Comparison of motor function after intralesional, intravenous transplantation of Bone Marrow Stromal Cells in rats of spinal cord injury : a Meta analysis

摘要 目的比较静脉移植与局部移植骨髓间充质干细胞(Bone Marrow Stromal Cells,BMSCs)对脊髓损伤大鼠运动功能恢复的效果。方法计算机检索Medline、EMbase、The Cochrane Library(2014年第10期)、中国生物医学文献数据库、万方数据库、中国知网1990~2014年第10期间的静脉移植与局部移植BMSCs治疗脊髓损伤大鼠的随机对照实验。使用Rev Man 5.2.0软件进行Meta分析,纳入研究分为脊髓损伤后3天内细胞移植(3天组)和脊髓损伤后7天细胞移植(7天组)2个亚组,对细胞移植后2、4、6周三个时间点的BBB评分进行分析。结果共6篇文献符合纳入标准。Meta分析结果示:细胞移植后2周,3天组静脉与局部移植BBB评分差异无统计学意义(WMD:-1.15[-2.72,0.42],=0.15),7天组静脉移植BBB评分低于局部移植,差异有统计学意义(WMD:-1.02[-1.99,-0.05],<0.05);细胞移植后4周,3天组静脉与局部移植BBB评分差异无统计学意义(WMD:-0.26[-2.26,1.74],=0.80),7天组静脉移植BBB评分低于局部移植,差异有统计学意义(WMD:-0.96[-1.85,-0.07],<0.05);细胞移植后6周,3天组静脉与局部移植BBB评分差异无统计学意义(WMD:2.56[-4.39,9.51],=0.47),7天组静脉移植BBB评分低于局部移植,差异有统计学意义(WMD:-1.72[-2.71,-0.73],<0.05)。结论脊髓损伤后3天内进行BMSCs移植,局部移植和静脉移植对改善大鼠后肢运动功能没有明显区别;脊髓损伤后7天进行移植,局部移植对大鼠后肢运动功能的恢复效果优于静脉移植。 Objective To compare motor function after intralesional ,intravenous transplantation of Bone Marrow Stromal Cells (BMSCs) in rats of spinal cord injury. Methods 6 studies were searched from Medline, EMbase, CNKI, The Cochrane Library, China Biology Medicinedisc and Wanfang data-base concerning intralesional, intravenous transplantation of BMSCs in treating spinal cord injury rats between 1990 and 2014 Oct, the data of the studies were collected and Meta-analyzed by RevMan 5.2.0 software, the studies were divided into 2 groups: within 3 days after spinal cord injury to transplant BMSCs (3 day group) and 7 days after spinal cord injury to transplant BMSCs (7 day group), and the BBB scale was meta-analyzed at 2weeks, 4weeks, 6weeks. Results 2 weeks after transplantation, no difference in BBB scale was found between intralesional and intravenous transplantation in 3 day group (WMD: - 1.15 [-2.72,0.42], P=-0.15), the BBB scale of intralesional transplantation was higher than intravenous transplantation in 7 day group (WMD: -1.02 [- 1.99, -0.05], P〈0.05); 4 weeks after transplantation, no difference in BBB scale was found between intralesional and intravenous transplantation in 3 day group (WMD: -0.26 [-2.26,1.74], P=-0.80), the BBB scale ofintralesional trans- plantation was higher than intravenous transplantation in 7 day group (WMD: -0.96 [-1.85, -0.07], P〈0.05); 6 weeks after transplantation, no difference in BBB scale was found between intralesional and intravenous transplantation in 3 day group (WMD: 2.56 [-4.39,9.51], P=0.47), the BBB scale ofintralesional transplantation was higher than intravenous transplantation in 7 day group (WMD: -1.72 [-2.71, -0.73], P〈0.05).
