摘要
目的:研究肿瘤坏死因子(TNF-α)在勃起功能障碍(Erectile dysfunction,ED)患者血清中的表达及意义,探讨ED患者血清中TNF-α的浓度变化与病情严重程度的相关性及其在ED诊断中的应用价值,探索血管内皮损害在ED发病机制中的作用,为ED的预防和治疗提供借鉴。方法:随机抽取门诊ED患者140例为试验组,按国际勃起功能评分表(IIEF-5)分为轻度组、轻至中度组、中度组、重度组,另选取正常体检的健康人30例为对照组,观察不同分组的ED患者TNF-α在血清中的表达。结果:轻度组、轻至中度组、中度组、重度组的ED患者血清TNF-α的异常表达例数,均多于对照组,差异均有统计学意义(X^2=15.52,P<0.001;X^2=13.22,P<0.001;X^2=47.78,P<0.001;X^2=28.12,P<0.001)。轻度组、轻至中度组、中度组、重度组的ED患者血清TNF-α均呈现高表达,与对照组比较,差异均有统计学意义(t=2.18,P=0.033;t=3.14,P=0.002;t=9.04,P<0.001;t=6.49,P<0.001)。结论:ED患者血清中TNF-α有着较高的表达并与ED的严重程度呈正相关,提示血管功能障碍可能与ED的发生和进展有着密切的联系。
Objective：To study the TNF-α in erectile dysfunction（ED） expression and significance in serum,to investigate the correlation between concentration and severity of TNF-α in patients with ED severity and its application value in the diagnosis of ED,explore the role of intravascular skin damage in the pathogenesis of ED,to provide reference for the prevention and treatment of ED.Method：140 patients in the outpatient department with ED were randomly divided into mild group,mild to moderate group,moderate group and severe group,according to the international erectile function score（IIEF-5）,30 health examination people were selected as control group,the expressions of TNF-α in the serum of ED in different groups were observed.Result：The number of abnormal serum TNF-α expression of the mild group,mild to moderate group, moderate group,severe group were more than that of the control group, the differences were statistically significant（ χ2=15.52,P〈0.001; χ2=13.22,P〈0.001; χ2=47.78,P〈0.001; χ2=28.12,P〈0.001）.The serum TNF-α showed high expression of the mild group,mild to moderate group,moderate group,severe group,compared with the control group,the differences were statistically significant（t=2.18,P=0.033;t=3.14,P=0.002;t=9.04,P〈0.001;t=6.49,P〈0.001）.Conclusion：ED in serum of patients with TNF-α has a high expression and positive correlation with the severity of ED,suggesting that vascular dysfunction may be associated with the occurrence and progression of ED.
