摘要
目的分析锥体束华勒变性多层螺旋CT及低场MRI影像特点,提高诊断水平。方法回顾性分析33例经临床及影像诊断的脑部华勒变性患者的临床及影像资料,根据病程分期,按CT、MRI分组进行对比。结果脑部华勒变性最常见于皮质脊髓束及皮质核束走行区,在原发病灶基础上,多层螺旋CT表现为远离原发病灶之锥体束区密度减低影,低场MRI表现为长T1、长T2信号,晚期患者出现病侧大脑脚、丘脑、基底节及半卵圆中心的萎缩。33例病例中,多层螺旋CT显示23例,一期2例,二期3例,三期10例,四期8例。低场MRI显示33例,一期4例,二期5例、三期16例、四期8例。其中低场MRI对早期(一~三期)华勒变性的诊断率优于CT。结论锥体束华勒变性具有一定的影像特征,多层螺旋CT及低场MRI对其诊断具有较高价值。
Objective To analyze the multi sclice spiral CT and low field MRI imaging manifestations of Wallerian degeneration for the improvement of its diagnosis.Methods The imaging and clinical data of 33 patients with brain Wallerian degeneration were divided into different groups according to the four stages and imaging findings of Multi-slice spiral CT and Low-field MRI.Results Wallerian degeneration mostly occurred in the corticospinal tract and tractus corticonuclearis in brain.With the primary lesion,CT showed Low density shadow in pyramidal tract area,MRI showed low T1 signal and high T2 signal in pyramidal tract area.In advanced patients the ipsilateral caudex cerebri,thalamus,basal ganglia,centrum ovale atrophy.In 33 patients,CT showed 23 cases,2 cases of first stage,3 cases of second stage,10 cases of third stage,8 cases of fourth stage.MRI showed 33 cases,4 cases of first stage,5 cases of second stage,16 cases of third stage,8 cases of fourth stage.The first stage-third stage have statistical significance.Conclusion Wallerian degeneration in brain has a certain image features,CT and MRI have high value for the diagnosis.MRI can make an early diagnosis of Wallerian degeneration in brain.
