摘要
目的对高龄胆总管结石应用内镜逆行胰胆管造影(ERCP)治疗的临床疗效进行分析。方法选取本院2010年1月至2016年1月诊治的99例高龄胆总管结石患者作为研究对象,随机分为两组,研究组50例采用ERCP治疗,对照组49例采用传统手术治疗,比较两组临床疗效。结果研究组的手术时间、黄疸消退时间、腹痛缓解时间、抗感染治疗时间、术后卧床时间以及住院时间均明显短于对照组,差异有统计学意义(t=27.96、2.88、1.54、2.54、2.67、6.65,P均<0.05);研究组的IL-1、TNF-α、IL-6等炎症反应均明显低于对照组,差异有统计学意义(t=12.33、3.73、13.43,P<0.05);研究组的手术成功率为92.00%,显著高于对照组的73.47%(P<0.05);且并发症发生率(10.00%)显著低于对照组的28.56%(P<0.05)。结论高龄胆总管结石应用ERCP治疗的临床效果优于传统手术治疗。
Objective To analyze the clinical effect of ERCP in the treatment of elderly patients with common bile duct stones. Methods Ninety nine elderly patients with common bileductstones were selected from January 2010 to January 2016 in our hospital and randomly divided into two groups. The study group （50 cases） received the ERCP treatment, while the control group （49 cases） received the traditional surgery. The curative effectwas compared between two groups. Results The operation time,jaundice fade time, abdominal pain relief time, anti-infection treatment time,postoperative bedtime and hospitalstay were significantly shorter than the control group, the differences were statistically significant （ t = 27.96, 2.88, 1.54, 2.54, 2.67, 6.65, P 〈 0.05）. The levels of IL-1, TNF-α,IL-6 in the study group were significantly lower than those in the control group, the differenceswere statisticallysignificant （ t = 12.33, 3.73,13.43, P 〈0.05）. The operation successrate in study group was92% （46/50） whichwas significantly higher thanthat in control group 73.47% （36/49） （ P 〈 0.05）. The incidence of complications in study group was 1 0 % （5/50） which was significantly lower than that in the control group 28.56% （14/49） （ P 〈 0.05）. Conclusion The clinical effect of ERCP in the treatment of elderly patients with common bile duct stones is better than that of traditional surgery.
