摘要
目的 调查淮南地区乙肝患者重叠感染丙肝的现状 ,并评价重叠感染丙肝对乙肝患者肝功影响。方法 ELISA法检测不同性别、年龄、类型的乙肝患者血清抗HCV -IgM和ALT、AST、AST/ALT及A/G比值。 结果 共检查乙肝患者血清标本 5 84份 ,抗HCV -IgM阳性率 7 5 3%(4 4/ 5 84)。其中男 7 19%(2 4/ 34 4) ;女8 33%(2 0 / 2 40 ) (X2 =0 . 2 0 ,P >0 . 0 5 )。各年龄组阳性率为 5 17%～ 10 %不等 ,统计学处理无差异。不同型乙肝血清抗HCV -IgM阳性率分别为 :急性黄疸型乙肝 5 5 3%、慢性肝炎 9 6 2 %、肝硬化 10 87%、慢性重症型乙肝 2 5 0 0 %。重叠感染丙肝的乙肝患者血清AST、ALT、AST/ALT、A/G依次为 96± 12、 6 6± 2 7、 1 3±0 5、 1 4 9± 0 36 ,与单纯乙肝患者相比差异显著 (P <0 . 0 1,P <0 . 0 5 )。结论 乙肝患者血清抗HCV—IgM阳性率较高 ,且与年龄、性别无关 ,似有随病程迁延阳性率增高趋势。机体重叠感染HCV可影响体内氨基酸代谢和蛋白质合成 ,影响肝脏损伤病灶的修复和肝细胞再生 ,临床表现为患者肝功能恢复时间延长。提示临床治疗乙肝患者 ,定期检测其血清抗HCV -IgM ,重视重叠感染的发生。
Objective To detect the positive rates of anti-HCV-IgM in serum of different patients with hepatitis B and its characteristics in Huainan area and to asses the liver function of the patients with hepatitis B whose serum anti-HCV-IgM is positive. Methods Anti-HCV-IgM in serum of patients with each type hepatitis B in different sexes and ages was detected by ELISA,at the same time, each patient was detected for ALT, AST,AST/ALT and A/G. Results There was 44 positive subjects (male 24, female 20) in 584 serum and the positive rate was 7.53%. The positive rates of male and female were 7.19%(24/344). 8.33%(20/240) respectively (X 2=0.20,P>0.05). No significant difference was found in positive rates(from5.17% to 10%) of different aged subjects. The positive rates of patients with acute ictero hepatitis, chronic hepatitis, cirrhosis and chrinic severe hepatitis were 5.53%,9.62%, 10.87%, 25.00%. The levels of ALT,AST,AST/ALT and A/G of each anti-HCV-IgM-positive subject were 96±12,66±27,1.3±0.5,1.49±0.36 respectively. There was significant difference between the experimental group and the control group. Conclusion The positive rates of anti-HCV-IgM in serum of patients with hepatitis B was high, which was not related to genders and ages, and associated with development of disease course. It is suggested that anti-HCV-IgM in serum of patients withhepatitis B can, be detected terminally.
