摘要
目的观察宫颈鳞癌细胞中乏氧诱导因子-1α(HIF-1α)的表达情况及其与预后的关系。方法通过治疗前检测119例宫颈鳞癌患者HIF-1α的表达情况,并分析阳性、阴性表达组放化疗疗效并研究其与宫颈癌预后的关系。结果 119例中晚期宫颈鳞癌患者中HIF-1α表达阳性者86例(72.3%),表达阴性者33例(27.7%)。Ⅱ、Ⅲ期患者表达阴性者的有效率(94.7%、85.7%)高于表达阳性者(87.8%、75.6%),差异有统计学意义(P=0.032,P=0.041);表达阴性者(89.4%、78.5%)2年局控率高于表达阳性者(75.5%、70.2%),差异有统计学意义(P=0.021、P=0.035);表达阴性者2年生存率(100%、85.7%)高于表达阳性患者(91.2%、72.9%),差异有统计学意义(P=0.034、P=0.041)。结论大部分中晚期宫颈鳞癌细胞HIF-1α表达阳性,对于Ⅱ、Ⅲ期患者HIF-1α表达阳性与不良预后相关。
Objective To explore the relationship between the expression of hypoxia-inducible factor-1 alpha（ HIF-1α） in locally advanced cervical cancer cells and this disease prognosis. Methods A total of 119 patients with stage II-III cervical squamous cancer treated with concurrent chemo-radiotherapy were enrolled in this study. The expression of HIF-1α was detected by immunohistochemistry. The correlation between HIF-1α expression and disease prognosis was analyzed. Results The number of HIF-1α positive expression was 86 cases（ 72.3%）,and the number of HIF-1α negative expression was 33 cases（ 27.7%）.For stage Ⅱ and Ⅲ patients,the effective rate in HIF-1α negative and positive expression groups after treatment was（ 94. 7%,85. 7%） and（ 87. 8%,75. 6%）,respectively. The difference was statistically significant（ P = 0.032,P = 0.041）. The local control rate after 2 years of treatment in HIF-1α positive group was significantly higher than that in the HIF-1α negative expression group（ 89. 4%,78. 5% vs 75. 5%,70.2%,P = 0.021,P = 0.035）. The survival rate after 2 years of treatment in HIF-1α negative expression group was significantly higher than that in the HIF-1α positive expression group（ 100%,85.7% vs 91.2%,72.9%,P = 0. 034,P = 0. 041）. Conclusion HIF-1α is positively expressed in most of cervical squamous cancer cells,which is correlated with poor prognosis in stage Ⅱ and Ⅲ patients.
