摘要
目的探讨大剂量盐酸氨溴索对重症烧伤肺功能保护的临床效果。方法 52例进行重症烧伤肺功能保护治疗的患者,随机分成观察组和对照组,各26例。观察组给予大剂量盐酸氨溴索治疗,对照组给予常规剂量盐酸氨溴索治疗。对比分析两组的治疗效果。结果治疗后,两组血氧饱和度(SpO_2)和动脉血二氧化碳分压(PaCO_2)比较差异无统计学意义(P>0.05),但动脉血氧分压(PaO_2)和呼吸机辅助时间比较差异具有统计学意义(P<0.05)。治疗后,观察组肺部损伤程度Ⅰ级18例、Ⅱ级6例、Ⅲ级2例,对照组分别为11、4、11例,两组肺部损伤程度比较差异具有统计学意义(P<0.05)。结论大剂量盐酸氨溴索的临床使用效果比较好,使肺功能和血气分析得到了明显的改善,大剂量治疗效果要比常规剂量治疗效果明显,没有出现比较突出的不良反应,因此大剂量盐酸氨溴索治疗因烧伤导致的肺损伤值得普遍推广。
Objective To investigate clinical effect by large dose of ambroxol hydrochioride for pulmonary function protection in severe burn.Methods A total of 52 patients receiving pulmonary function protection in severe burn were randomly divided into observation group and control group,with 26 cases in each group.The observation group received large dose of ambroxol hydrochloride for treatment,and the control group received conventional dose of ambroxol hydrochloride for treatment.Comparative analysis was made on curative effects between the two groups.Results After treatment,there was no statistically significant difference of pulse oxygen saturation(SpO_2) and arterial partial pressure of carbon dioxide(PaCO_2) between the two groups(P〈0.05),while the difference of arterial partial pressure of oxygen(PaO_2) and ventilator auxiliary time had statistical significance(P〈0.05).After treatment,the observation group had 18 cases with grade Ⅰ pulmonary injury,6 cases with grade Ⅱ,and 2 cases with grade Ⅲ,and the contrd group had respectively 11,4,11 cases.The difference of pulmonary injury degree had statistical significance between the two groups(P〈0.05).Conclusion Largedoseof ambroxd hydrochloride shows excellent effect in clinical application,and it provides remarkable improvements in pulmonary function and blood gas analysis.Implement of large dose provides better curative effect than conventional dose,without obvious adverse reactions.Therefore,large dose of ambroxd hydrochloride is worth widely promoiting for treating severe burn- induced pulmonary injury.
