摘要
目的:探讨氯诺昔康对老年髋关节置换术后皮下患者自控镇痛的效果及不良反应,为临床用药提供参考。方法:选择2014年1月—2016年1月南阳市骨科医院收治的拟行全髋关节置换术或半髋关节置换术患者87例,以随机数字表法分为观察组45例、对照组42例。对照组患者术后给予舒芬太尼自控镇痛,观察组患者在对照组基础上联合应用氯诺昔康自控镇痛,比较2组患者术后不同时间点(术后1、2、4、8、24、48 h)疼痛视觉模拟评分、舒适度评分及术后不良反应发生情况。结果:术后各时间点,观察组患者的疼痛视觉模拟评分均显著低于对照组,舒适度评分均显著高于对照组,差异均有统计学意义(P<0.05);观察组患者不良反应发生率为15.56%(7/45),对照组不良反应发生率为14.29%(6/42),2组的差异无统计学意义(P>0.05)。结论:老年髋关节置换患者术后在舒芬太尼基础上联合应用氯诺昔康皮下自控镇痛,能有效缓解术后疼痛,提高术后舒适度,且用药安全。
OBJECTIVE： To probe into the effects and adverse drug reactions of lornoxicam on patient controlled subcutaneous analgesia in elderly hip replacement patients, so as to provide reference for clinical drug use.METHODS： 87 cases of total hip replacement or half total hip replacement admitted into Nanyang Orthopaedic Hospital from Jan. 2014 to Jan. 2016 were randomly selected to be divided into observation group and control group via the random number table,with 42 cases in control group and 45 cases in observation group. The control group were treated with sufentanil analgesia after surgery,while the observation group additionally received lornoxicam analgesia based o the control group. The visual analogue scale（ VAS）,bruggrmann comfort scale（ BCS） and adverse drug reactions at different time points（ after surgery of 1 h,2 h,4 h,8 h,24 h and 48 h） of two groups were compared. RESULTS：After surgery,the VAS score of the observation group was significantly lower than that of the control group at different time points,while the BCS score of the observation group was significantly higher than that of the control group,with statistically significant difference（ P〈0. 05）. The incidence of adverse drug reactions in observation group was15. 56%（ 7 /45）,and in control group was 14. 29%（ 6 /42）,there was no significant difference between two groups（ P〉0. 05）. CONCLUSIONS： The use of lornoxicam for patient controlled subcutaneous analgesia in elderly hip replacement patients can effectively relieve the postoperative pain,improve patients＇ comfort with safe medication.
