摘要
目的:调查清远市妇幼保健院(以下简称"我院")门、急诊处方抗菌药物使用情况,探讨用药合理性,为门诊合理应用抗菌药物提供参考依据。方法:参照《处方管理办法》《新编药物学》(17版)、《抗菌药物临床应用指导原则》《抗菌药物临床应用管理办法》,以《医院处方点评管理规范(试行)》为标准,随机抽取我院2015年1月—2016年7月1 200张门、急诊抗菌药物处方进行统计分析,评价其用药合理性。结果:抗菌药物的临床使用以单一用药为主,占89.33%(1 072/1 200),二联用药占10.67%(128/1 200),未发现三联用药;β-内酰胺类抗菌药物使用率最高,为63.45%(854例次/合计1 346例次),其次为大环内酯类。1 200张抗菌药物处方中,不合理用药处方57张,不合理用药率为4.75%,主要为无指征应用抗菌药物、联合用药不适宜、溶剂不适宜等。结论:我院门、急诊抗菌药物的使用基本合理,但仍有不合理用药情况,应引起临床重视,以提高抗菌药物合理使用水平。
OBJECTIVE： To investigate the application status of antibiotics in outpatient and emergency prescriptions in Qingyuan Maternity and Child Care Hospital（ hereinafter referred to as ＂our hospital＂）,and to probe into the rationality,so as to provide reference for the rational application of antibiotics in outpatient department.METHODS： A total of 1 200 outpatient prescriptions of our hospital were randomly selected and statistically analyzed with regard to the application rationality of antibiotics in accordance with ＂Prescription Management＂,＂Newly Compiled Pharmacology＂,＂Administrative Measures for Clinical Use of Antibiotics＂,＂Guiding Principles for Clinical Use of Antibiotics ＂, with ＂Hospital Prescription Review Management Practices（ trial） ＂ as reference criteria.RESULTS： The clinical application of antibiotics mainly concentrated on single medication,accounting for 89. 33%（ 1 072 /1 200）; the combination of two drugs was 10. 66%（ 128 /1 200）,and there was no combination of three drugs. The application rate of β-lactam antibiotics took the lead,accounting for 63. 45%（ 854 /1 346）,followed by macrolide. Of the 1 200 antibiotics prescriptions,irrational prescriptions were 57 cases,and the irrational application rate was 4. 75%,mainly performed as no indications for the antibiotics,improper drug combination and solvent,etc.CONCLUSIONS： The application of of antibiotics in outpatient and emergency department of our hospital was basically rational,yet there are still some irrational phenomenon. It is necessary for the clinic to attach more importance to the phenomenon,so as to improve the rational application level of antibiotics.
