摘要
目的：评价斜仰卧位经皮肾镜取石术（percutaneous nephrolithotomy,PNL）治疗上尿路结石的疗效和安全性。方法：回顾性分析2014年1月至2015年12月常熟市第二人民医院94例斜仰卧位行PNL的上尿路结石患者的临床资料,分析患者术后结石残留、出血、感染情况。结果：94例患者均顺利完成手术。术后一期清除结石90例,二期清除结石4例;需输血患者5例,其中保守治疗无效行肾动脉数字减影血管造影（digital subtraction angiography,DSA）栓塞术止血1例;尿脓毒血症8例,经抗感染治愈;无败血症休克病例。随访2个月-2年,患者均健在,无结石复发需二次手术患者。结论：斜仰卧位PNL一期结石清除率较高,并发症可控,对于上尿路结石的治疗安全、有效。
Objective ： To evaluate the efficacy and safety of percutaneous nephrolithotomy （PN L） in oblique supineposition for the treatment of upper urinary tract stones. Methods ： A total of 94 patients with upper urinary tract stones weretreated with PNL from January 2014 to December 2015 in Changshu No. 2 People’ s Hospital. Their clinical data were analyzedretrospectively and patients？ postoperative residual stone, bleeding and infection were recorded. Results： PNL was completedsuccessfully in all 94 cases. Stones were cleared in 90 cases at the first stage and 4 cases at the second stage. Five patientsrequired blood transfusion, and one of them underwent renal artery digital subtraction angiography （DSA） embolization to stopbleeding after the conservative treatment was ineffective. Eight cases of urinary sepsis were cured by anti-infection therapy, andno septic shock occurred. From 2 to 24 months follow-up, all patients were alive, and no patients needed further surgerybecause of stone recurrence. Conclusions： The rate of one stage stone clearance of PNL in the oblique supine position is high,and the complications are controllable, so it is safe and effective in treating upper urinary tract stones.
