摘要
目的:研究经皮肾镜碎石术(percutaneous nephroithotomy,PCNL)后发生全身炎症反应综合征(systemic inflammatory response syndrome,SIRS)的影响因素以便更好的指导临床如何预防PCNL后SIRS的发生。方法:收集重庆医科大学附属第一医院2012年12月至2014年1月121例行PCNL患者,通过回顾性分析研究年龄、结石的位置、性别、肾脏积水程度、肾脏结石大小、手术时间、结石培养、肾盂尿培养、术前中段尿培养与术后是否发生SIRS的关系。单因素分析采用卡方检验,多因素分析采用logistic回归分析。结果:单因素分析结果提示性别、肾脏积水程度、肾脏结石大小、手术时间、结石培养、肾盂尿培养、术前中段尿培养与术后发生SIRS有一定关系(P<0.05),与年龄及结石位置无关系(P>0.05),多因素研究结果表明:术后SIRS的发生与手术时间、性别、结石培养及肾脏积水程度有密切关系(P<0.05)。结论:PCNL术后SIRS的发生与手术时间、性别、结石培养及肾脏积水程度有密切关系,需引起高度重视。PCNL术中应严格控制手术时间,术后需完善结石细菌培养。
Objective：To analyze the related influencing factors which cause systemic inflammatory response syndrome（SIRS） after percutaneous nephroithotomy（PCNL） in order to get better clue to the prevention. Methods：Medical records of 121 patients who treated with PCNL from December 2012 to January 2014 in the First Affiliated Hospital of Chongqing Medical University were reviewed retrospectively according to age,side（left vs.right）,gender（male vs.female）,level of hydronephrosis,size of stones,time of surgery,stone culture,pelvic urine culture and preoperative midstream urine culture.The single factor was analyzed by Chi-square test and multi-factor was analyzed by logistic regression analysis. Results：Single factor analysis indicated that gender,level of hydronephrosis,size of stones,operation time,stone culture,pelvic urine culture and preoperative middle urine culture had certain relationship with postoperative SIRS（P〈0.05）;age and side had no relationship with postoperative SIRS（P〈0.05）. Multiple-factor analysis indicated that gender,level of hydronephrosis,operation time,stone culture had certain relationship with postoperative SIRS（P〈0.05）.Conclusion：The influence factors of SIRS after PCNL include gender,level of hydronephrosis,time of surgery,stone culture,which should been taken seriously. We should control operation time and improve the bacteria culture of the stone after the operation.
