摘要
目的:研究肛门乳头状汗腺瘤的临床病理特征。方法:收集华山医院宝山分院病理科2001年-2014年病理诊断为肛门乳头状汗腺瘤资料,采用常规HE染色及免疫组织化学染色进行分析。免疫组织化学抗体包括:HCK、EMA、ER、PR、CK7、S100、Calponin、p63、CD10、SMA、MSA、GCDPF-15及CK20。结果:4例肛门乳头状汗腺瘤均为女性,平均年龄47岁。临床表现为直径0.5~1.5厘米大小的皮下肿物;病理巨检为灰白色、质地较硬的类圆形肿物;镜下见肿瘤位于真皮内,肿瘤边缘为纤维及玻璃样间质,界清,与表皮不相连,肿瘤图像类似于乳腺导管内乳头状瘤,为迂回曲折、复杂折叠的乳头结构组成,乳头纤细,中央为血管纤维间质,乳头表面双层细胞结构排列,内层为单层柱状上皮,外层为单层立方肌上皮细胞。4病例细胞形态均比较均一。1例细胞增生活跃伴轻度不典型增生。免疫组化显示为:内层柱状上皮表达ER、PR、HCK、CK7、EMA,外层肌上皮表达p63、CD10、S100、Calponin、MSA、SMA。结论:肛门乳头状汗腺瘤多发于女性,是比较少见的皮肤良性肿瘤,临床表现为痔疮出血、肛瘘等,容易误诊。预后佳,随访显示肿瘤未复发。
Objective: To investigate the clinicopathologic features of hidradenoma papilliferum. Methods: The clinical and pathological data were reviewed and studied in four cases of hidradenoma papilliferum. All cases were stained by HE and immunohistochemical,the antibodies including CK- pan,EMA,ER,PR,CK7,S100,Calponin,p63,CD10,SMA,MSA,GCDPF- 15 and CK20. Results: There were four female and the mean age was 47. The tumors diameter were from 0. 5cm to 1. 5cm. The papillary structures were present in the tumor. The papillary structure had a central fibrovascular core. the lumina of those structures were usually lined with a double layer of cells consisting of a luminal layer of columnar epithelial cells that showed active decapitation secretin similar to that seen in secretory cells of apocrine glangs and an outer one or two layers of cuboidal flattened myoepithelial cells. 4 cases were relatively uniform cells form. 1 case of active cell hyperplasia with mild dysplasia. Immunohistochemical display for the epithelia cells of all the tumors were positive for ER,PR,HCK,CK7,EMA,the myoepithlial cells of all cases were positive for p63,CD10,S100,Calponin,MSA,SMA. Conclusion: The anus hidradenoma papilliferum is a rare bengin tumor,more in famle. Clinical characteristics such as bleeding hemorrhoids,anal fistula,easily misdiagnosed. No recurrence is observed after a complete surgical excision.
