醒脑静注射液联用高压氧治疗一氧化碳中毒的临床效果 被引量:19

摘要 目的 探讨醒脑静注射液联用高压氧治疗一氧化碳中毒的临床效果。 方法 选取2011年10月至2013年10月于陕西省人民医院就诊的80例一氧化碳中毒患者进行研究 。完全随机分为观察组和对照组,各40例。对照组常规治疗基础上进行高压氧治疗,观察组在对照组的基础上给予醒脑静注射液20 ml加入5%葡萄糖注射液静脉滴注,1次/d,2组均 治疗15 d,治疗后随访3个月。采用格拉斯哥昏迷指数(GCS)评价患者治疗前和治疗后24 h的昏迷程度,观察2组患者的临床疗效和迟发性脑病发生情况。 结果 观察组患者总 有效率[97.5%(39/40)]明显高于对照组[80.0%(32/40)],差异有统计学意义(P〈0.05);2组患者治疗后的GCS评分明显高于治疗前,且观察组明显高于对照组[观察组治 疗前后分别为(4.2±1.6)、(14.9±1.8)分,对照组分别为(4.4±1.3)、(10.6±1.7)分],差异均有统计学意义(均P<0.05);观察组迟发性脑病发生率[2.5%(1/40) ]明显低于对照组[15.0%(6/40)],差异有统计学意义(P〈0.05)。 结论 醒脑静注射液联用高压氧治疗一氧化碳中毒的临床效果明显,可以明显改善昏迷症状,降低迟发 性脑病发生率。 Objective To explore the clinical effect of Xingnaojing injection combined with hyperbaric oxygen therapy in treating carbon monoxide(CO) poisoning. Methods Totally 80 cases of CO poisoning from October 2011 to October 2013 in Shaanxi Provincial People′s Hospital were randomly divided into observation group and control group, with 40 cases in each group. The control group was treated with hyperbaric oxygen therapy; the observation group was treated with Xingnaojing injection(20 ml, mixed with 5% glucose injection, intravenous drip, 1 time/d) combined with hyperbaric oxygen therapy. Both groups were treated for 15 d and they were followed up for 3 months after treatment. The degree of coma was evaluated by Glasgow Coma Scale(GCS) before treatment and 24 h after treatment. Clinical effect and the occurrence of delayed encephalopathy were analyzed between groups. Results The total effective rate in observation group was significantly higher than that in control group [97.5%(39/40) vs 80.0%(32/40)](P〈0.05). After treatment, GCS scores were significantly higher than those before treatment in both groups[observation group: (14.9±1.8)scores vs (4.2±1.6)scores, control group: (10.6±1.7)scores vs (4.4±1.3)scores](P〈0.05), and the GCS score in observation group was significantly higher than that in control group(P〈0.05) . The incidence of delayed encephalopathy in observation group was significantly lower than that in control group[2.5%(1/40) vs 15.0%(6/40)](P〈0.05) . Conclusion Xingnaojing injection combined with hyperbaric oxygen therapy is effective in the treatment of CO poisoning; it can significantly reduce coma symptom and the incidence of delayed encephalopathy.
