摘要
目的分析脑梗死病人血清同型半胱氨酸(Hcy)水平与病情严重程度的关系。方法选取2013年10月—2015年10月我院收治的急性脑梗死病人60例及同期进行体检的健康者60名作为研究对象。采用发光免疫法检测脑梗死病人及健康体检者血清Hcy水平,应用多因素回归分析血清同型半胱氨酸水平与脑梗死病人病情严重程度的关系。结果 Hcy水平与肌酐水平呈显著正相关关系(t=2.734,P=0.006);脑梗死病人心源性卒中(CS)、大动脉粥样硬化(LAA)及其他病因(OE)分型病人血清中Hcy水平与健康对照组有统计学意义(t=6.553,P=0.003;t=15.953,P=0.000;t=5.697,P=0.000);脑梗死组病人中发病机制为低灌注/栓子清除率下降、载体动脉栓塞穿支动脉、动脉-动脉栓塞及混合机制的病人血清中Hcy水平显著高于健康对照组(t=5.215,P=0.003;t=11.918,P=0.000;t=6.066,P=0.000;t=4.016,P=0.028);不同病情程度病人血清Hcy水平差异具有统计学意义(F=13.485,P=0.000),病人血清Hcy水平与脑梗死严重程度呈显著性正相关(r=0.708)。结论脑梗死病人血清Hcy水平与病人病情严重程度存在明显的正相关关系。
Objective To investigate the correlation of serum homocysteine level with the severity of neurological deficit in patients with cerebral infarction.Methods Sixty patients with cerebral infarction were selected in October 2013 to October 2015.Sixty healthy individuals were as controls.The serum homocysteine level was detected by luminescence immunoassay.The correlation of serum homocysteine level with the severity of cerebral infarction was analyzed by multivariate analysis.Results Serum Hcy level was positively correlated with creatinine levels(t=2.734,P =2.734).There was significant difference in serum Hcy levels between cerebral infarction patients with cardiogenic stroke(CS),large artery atherosclerosis(LAA),other etiologies(OE)and healthy control group(t=6.553,P =0.003;t=15.953,P =0.000,t=5.697,P =0.000).Serum Hcy levels in patients with hypoperfusion/impaired emboli clearance,carrier artery embolism perforators,arteries-artery embolism and mixing mechanism were higher than that in healthy controls(t=5.215,P =0.003;t=11.918,P =0.000;t=6.066,P =0.000;t=4.016,P =0.028).There was significant difference in serum Hcy levels among different degree of severity(F =13.485,P =13.485).Serum Hcy level was positively correlated with the severity of cerebral infarction(r=0.708).Conclusion Serum Hcy level was positively correlated with the severity of cerebral infarction.
