Effect of Different Frequency Ice Stimulation on Dysphagia in Patients with Alzheimer' Disease

摘要 目的探讨不同频次冰刺激对阿尔茨海默病(Alzheimer’disease,AD)患者吞咽功能障碍的的影响。方法便利抽样选择2009年1月至2013年1月在某医院精神科住院的AD伴有吞咽功能障碍的患者60例,随机分为两组各30例,在常规护理基础上,分别予冰刺激治疗每天2次和每天3次,疗程均为6周;于治疗前、治疗后6周进行洼田饮水试验评分,同时测定饮水试验时血氧饱和度(SaO2)下降值以评估疗效。结果治疗后6周时,两组患者组内洼田饮水试验评分均高于治疗前、饮水试验时SaO_2下降值均低于治疗前(均P<0.05),组间洼田饮水试验评分、饮水试验时SaO_2下降值差异则无统计学意义(均P>0.05)。两组治疗有效率(分别为60%、67%)以及饮水试验时SaO_2下降值恢复至2%以下的患者比例(分别为53%、60%)差异无统计学意义(均P>0.05)。结论每日2次、3次冰刺激治疗对AD患者吞咽功能障碍的疗效相近,可根据患者需要选择使用。 Objective To discuss the effect of different frequency ice stimulation on dysphagia in patients with Alzheimer' disease (AD).Methods By convenience sampling, 60 cases were selected and randomly divided into two groups,with 30 cases in each group.On the basis of routine nursing,patients were received ice stimulation twice and three times a day respectively, and it lasted for 6 weeks.The effect was assessed with Kubota's water swallowing test before treatment and 6 weeks after treatment, and arterial oxygen saturation (SaO2) was tested to evaluate the efficacy.Results After treatment of 6 weeks,the scores of water swallowing test in both two groups were higher than before, the SaO2 score was lower than before (all P〈0.05).There was no statistical significance of water swallowing test and degrade of SaO2 between two groups (all P〉0.05).There was no statistical significance of the treatment efficacy (60% vs 67%)and the ratio (53% vs 60%)of improved degradation of SaO2 (decreased from ≥2% to〈2%) in two groups (all P〉0.05).Conclusions The ice stimulation therapy in twice or third daily had a similar treatment efficacy on the dysphagia in patients with Alzheimer' disease,it can he chosen by patients.
