摘要
目的了解宁夏银川、吴忠两市中小学生脂代谢相关指标的异常情况,为预防儿童相关慢性疾病的发生提供基础数据。方法在银川市兴庆区、金凤区、西夏区和吴忠市分层随机整群抽取小学、初中和高中在校学生1786名,对抽取的中小学生进行身高、体重的测量,采集空腹静脉血,测定总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)。结果银川、吴忠两市中小学生脂代谢相关指标TC,TG,LDL-C,HDL-C的异常检出率分别为1.5%,14.5%,0.4%,33.7%,男生LDL-C异常检出率(0.8%)高于女生(0.1%),差异有统计学意义(χ^2=4.825,P=0.028)。脂代谢相关指标年龄分布,TC,LDL-C异常集中在9-11岁,而TG,HDL-C异常检出率随年龄增长呈上升趋势;脂代谢水平比较,TC,LDL-C水平女生高于男生(t值分别为-2.602,-2.607,P值均〈0.05),TC,LDL-C,HDL-C随年龄的增长呈下降的趋势,TG则随年龄的增长呈上升趋势,4项指标在年龄间差异均有统计学意义(F值分别为13.541,13.813,18.738,20.327,P值均〈0.01)。回、汉族中小学生脂代谢异常检出率差异均无统计学意义;汉族学生TC水平高于回族学生,差异有统计学意义(t=2.990,P=0.003)。TC,TG,LDL-C异常检出率在不同体型学生间差异均有统计学意义,肥胖组和超重组学生的TC,TG,LDL-C水平均高于正常组学生(P值均〈0.05),且均随BMI增加而逐渐升高。结论银川、吴忠两市中小学生脂代谢相关指标的异常检出率不容乐观,超重和肥胖是中小学生脂代谢异常的影响因素之一。需重视中小学生肥胖的预防,对肥胖超重的学生进行脂代谢的相关检查和必要干预。
Objective To understand lipid abnormalities among children and adolescents in Yinchuan and Wuzhong. Methods All the participants were selected from elementary and junior schools in three districts (Xingqing district, Jinfeng District, Xixia District) of Yinchuan and Wuzhong city through cluster sampling. Height and weight was assessed. Total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) was measured. Results A total of 1 786 children and adolescents aged 7 to 18 years old, TC, TG, LDL-C and HDL-C abnormal detection rate was 1.5%, 14.5%, 0.4%, 33.7%, respectively. Abnormality in LDL-C was higher in boys (F= 4.825, P= 0.028). Abnormality in TC and LDC-C was more common in children aged 9-11 years. There was a significant increasing trend in TC, LDL -C and HDL-C abnormality with age. TC and LDL-C concentration was higher among girls (t=-2.602, -2.607, P〈0.05). Concentration of TC, LDL-C and HDL-C decreased with age, while TG increased with age ( F= 13.541, 13.813, 18.738 and 20.527, respectively, P〈0.01). No significant difference in lipid abnormalities in children with Han and Hui ethnicity. Lipid concentrations were higher among children with Han ethnicity compared with Hui children (t = 2.990, P = 0.003). TC, TG and LDL-C concentra- tion was significantly higher in children with overweight and obesity, while HDL-C lower than normal weight children. Conclusion Children and adolescents in Yinchuan and Wuzhong are at increased risk of developing dyslipidemia. Continued monitoring of lipid levels in children appears warranted, especially among overweight and obese individuals.
