摘要
目的探讨改良侧卧位在胃食管结合部肿瘤手术中的应用效果。方法将300例胃食管结合部肿瘤手术患者按手术时间分为对照组和改良组各150例,对照组采用传统仰卧位手术,改良组采用改良侧卧位手术。比较两组手术时间、术中出血量、术野暴露评分和淋巴结清扫难易程度评分。结果改良组手术时间和术中出血量显著少于对照组(均P<0.01),医生对术野暴露的满意度及淋巴结清扫难易评分显著高于对照组(均P<0.01)。结论改良侧卧位可以有效缩短胃食管结合部肿瘤手术的手术时间,减少术中出血量,充分暴露手术野,利于淋巴结清扫。
Objective To study the effect of modified lateral position in surgical management of gastroesophageal junction tumor. Methods A total of 300 patient with gastroesophageal junction tumor were subjected to a modified lateral position （n 150） or a tra ditional supine position （n=150） according to chronological sequence. The length of surgery, bleeding volume, exposure of the operative field, and difficulty in lymph node dissection were compared. Results The modified position group had shorter length of surgery, and smaller bleeding volume than the controls （P^0.01 for both） ; surgeons had higher satisfaction with exposure of the operative field for the modified position group, and they also had less difficulty in lymph node dissection （P（0.01 for both）. Con- clusion Modified lateral position in surgical management of gastroesophageal junction tumor could significantly shorten length of surgery, decrease bleeding volume, and promote exposure of the operative field the operating filed, which results in convenient lymph node dissection.
