摘要
目的研究大鼠舌黏膜癌变过程中外周血的细胞角蛋白19(CK19)和E-钙黏蛋白(E-cad)的表达及相关性。方法 90只SPF级SD雄性大鼠,按照随机数字表法分为6组,利用4NQO饮水法诱导75只大鼠(实验组)舌黏膜癌变,15只大鼠(对照组)饮用SPF级动物专用纯净灭菌水,于不同时期分批乙醚麻醉抽取外周血,颈椎脱臼处死取舌体组织做病理诊断,采用酶联免疫吸附试验(ELISA)检测外周血中CK19和E-cad在大鼠舌黏膜癌变过程中的动态变化,Pearson相关系数及线性回归模型分析E-cad与CK19相关性。结果 E-cad和CK19在正常黏膜、上皮单纯增生、轻度上皮异常增生、中重度上皮异常增生和舌鳞状细胞癌组外周血中浓度分别为(259.92±89.75)、(402.85±93.52)、(412.39±64.08)、(466.25±66.72)、(539.14±108.07)ng/L和(593.45±134.06)、(784.45±108.53)、(848.58±99.68)、(951.94±85.31)、(1 004.22±127.65)ng/L。上皮单纯增生、轻度上皮异常增生、中重度上皮异常增生和舌鳞状细胞癌组外周血中E-cad和CK19表达显著高于正常黏膜(P<0.05);E-cad和CK19的表达呈正相关(r=0.738,P<0.001)。结论外周血中CK19和E-cad表达上升是大鼠舌黏膜癌变过程中的早期事件。
Objective To evaluate the expression of serum cytokeratin 19 (CK19) and E - cadherin ( E - cad) in various stages of oral carcinogenesis and investigate the correlation between CK19 and E - cad in the process of oral cancer. Methods A total of 90 specific pathogen free (SPF) male Wistar rats were randomly divided into two groups: 4NQO - treated group ( n = 75, treatment of 40 parts per million [ ppm ] 4NQO solution) and healthy control group ( n = 15, treatment of water). The rats were sacrificed at different stages. The peripheral blood and tongue tissue samples were collected. The 5 μm thick sections were stained with hematoxylin and eosin for hlstopathological analysis. Enzyme - linked immunosorbent assay (ELISA) experimental method was used to study the expression of serum E -cad and CK19 in various stages of oral carcinogenesis. Pearson correlation coefficient and linear regression model were used to analyze the correlation between serum E - cad and CK19. Results The concentrations of CK19 and E - cad in normal group, epithelial hyperplasia group, mild dysplasia group, moderate - severe epithelial dysptasia group and squamous cell carcinoma group were (259.92 ± 89.75) and (402. 85 ±93.52) ng/L, ( 412, 39 ± 64.08 ) and (466.25± 66.72) rig/L, ( 539.14 ±108.07) and (593.45 ±134. 06) ng/L, (784. 45 ± 108.53 ) and ( 848.58 ± 99. 68) ng/L, and (951.94±85.31 ) and (1 004. 22± 127.65)ng/L, respectively. There were significant differences between normal and epithelial hyperplasia, mild dysplasia, moderate -severe epithelial dysplasia or squamous cell carcinoma, in both E -cad and CK19 expression (P 〈 0. 05). The contents of CK19 and E- cad in peripheral blood were positively correlated (r = 0. 738, P 〈 0. 001 ). Conclusion The up - regulation expression of CK19 and E - cad in peripheral blood is an early event in rat tongue mucosa carcinogenesis.
