摘要
目的探讨应用目标设定改善自我管理在慢性心力衰竭患者中的应用效果。方法目的抽样选择在2013年3月至2014年3月,上海市某三级医院心内科住院治疗的94例慢性心力衰竭患者为研究对象,按照患者入住病房的奇数偶数房间号将其分为目标设定组(n=45)和传统组(n=49),目标设定组给予目标设定干预和传统护理,传统组仅进行传统的护理措施。出院6个月后比较两组患者的自我管理水平、再次住院率、死亡率、采用6分钟步行试验评价心功能。结果目标设定组综合护理能力、心衰知识知情性、自我护理行动力、自我保健的能力均优于传统组,差异有统计学意义。目标设定组患者再入院率、病死率低于传统组,分别为10.2%vs 28.9%、2.0%vs 6.7%,6min步行试验优于传统组,为429.2±49.5m vs301.5±39.1m,差异有统计学意义。结论应用目标设定加强慢性心力衰竭患者的指导,改善患者的预后和心功能,这可能与其改善患者的综合护理能力有关。
Objective To explore the effects of self -management improved by goal setting on patients with chronic heart failure.Methods Purposive sampling was used to select 94 patients with chronic heart failure from the Department of Cardiology in a tertiary hospital in Shanghai from March , 2013 to March, 2014.The pa-tients were divided into goal setting group （n =45） and tradition group （n =49） , according to odd and even num-ber of the wards.Patients in goal setting group were given both goal setting and traditional nursing treatment , while patients in tradition group were given only traditional nursing treatment .The level of self -management, readmis-sion rate, mortality and 6 -minute walk test were measured after 6 months.Results Compared to tradition group, goal setting group showed better results in comprehensive nursing ability , knowledge of heart failure, self -nursing ability and self -care ability, which are statistically significant.Goal setting group also showed lower readmission rate （10.2% vs 28.9%）, lower mortality （2.0% vs 6.7%） , and better result in 6 -minute walk test （429.2 ± 49.5m vs 301.5 ±39.1m）, with statistical significance.Conclusion Self -goal setting strengthened the capabili -ty of patients with chronic heart failure and improved their prognosis and cardiac function , which may be associated with comprehensive nursing ability .
