摘要
目的观察当归补血汤辅助治疗糖尿病肾病(DN)的效果。方法将88例DN患者随机分为2组,对照组43例进行常规基础治疗,治疗组45例在对照组治疗基础上加当归补血汤,2组疗程均为2个月。检测2组治疗前后肾功能指标24 h尿蛋白定量、尿白蛋白(Alb)、内生肌酐清除率(CCr)、血肌酐(SCr)、血尿素氮(BUN),血糖指标空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(Hb A_1c),血脂指标血清胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)和高密度脂蛋白胆固醇(HDL-C)变化情况。结果2组治疗后肾功能指标24 h尿蛋白定量、Alb、SCr、BUN,血糖指标FPG、2 h PG,血脂指标TC、TG及LDLC均较本组治疗前降低(P<0.05),HDL-C升高(P<0.05),且2组各肾功能指标及血脂指标、2 h PG组间比较差异均有统计学意义(P<0.05)。2组治疗前后CCr、Hb A_1c均无明显变化(P>0.05)。结论当归补血汤能够调节脂质代谢紊乱,改善肾脏微循环,降低DN蛋白尿效果明显。
Objective To observe the effects of Danggui - buxue decoction on the treatment of diabetic ne-phropathy （ DN） . Methods 88 DN patients were randomly divided into two groups. 43 cases in control group were treated by routine basic treatment, and 45 cases in treatment group were treated by Danggui - buxue decoction on the basis of control group treatment, continuously treatment for two months. The 24 h urine protein quantitation, urinary albumin （ Alb）,endogenous creatinine clearance （ CCr）,serum creatinine（SCr） , blood urea nitrogen （BUN） , fast-ing blood - glucose （ FPG） , 2 h postprandial blood glucose （2 hPG）,glycosylated hemoglobin （HbA^）,serum cholesterol （TC）,triglyceride （TG）,low density lipoprotein cholesterol （LDL-C） and high density lipoprotein cholesterol （HDL-C） before and after treatment were measured in two groups. Results The 24 h urine protein quantitation, Alb, SCr,BUN,FPG, 2 hPG, TC, TG and LDL - C after treatment were decreased in two groups （P 〈 0. 05 ） , and the HDL - C was increased （ P 〈 0. 05 ） , there were statistical differences on indexes of the renal func-tion and blood lipid,2 hPG between two groups （P 〈0. 05）. There were no obviously changes on CCr and HbA^ be-tween before and after treatment in two groups （ P 〉 0. 05 ） . Conclusion Danggui - buxue decoction can regulate lipid metabolism disorders, improve renal microcirculation, has significant effects of reducing DN proteinuria.
