摘要
目的观察复方异丙托溴铵联合布地奈德对支气管哮喘急性发作期患儿肺功能及机体炎性反应的影响。方法选取该院收治的支气管哮喘急性发作期患儿70例作为研究对象,按照随机数字表法分为对照组和研究组各35例,对照组采用布地奈德治疗,研究组在对照组基础上联合复方异丙托溴铵治疗,观察两组患儿临床治疗效果、肺功能、炎性反应及不良反应发生率。结果研究组治疗总有效率为91.43%,高于对照组的71.43%,差异有统计学意义(P<0.05);治疗后两组患者第1秒末用力呼气量、用力肺活量、25%最大呼气流速、最大呼气峰流速水平均较治疗前升高,差异均有统计学意义(P<0.05),且研究组较对照组显著升高,差异也有统计学意义(P<0.05);治疗后两组患者白细胞介素-6、白细胞介素-8、白细胞介素-13、肿瘤坏死因子-α水平均较治疗前降低,差异有统计学意义(P<0.05),且研究组较对照组显著降低,差异也有统计学意义(P<0.05);研究组不良反应发生率为14.29%,低于对照组的17.14%,但差异无统计学意义(P>0.05)。结论采用复方异丙托溴铵联合布地奈德治疗支气管哮喘急性发作期患儿可有效改善肺功能及机体炎性反应,提高疗效。
Objective To observe the effect of compound ipratropium bromide combined with budesonide on lung function and inflammatory response in children with acute attack of bronchial asthma.Methods Seventy cases of children with acute attack of bronchial asthma treated in our hospital were selected as the research subjects,who were divided into control group and study group according to the random digital table method,with thirty-five cases in each group.The control group were treated with budesonide,while the study group were treated with compound ipratropium bromide based on the control group.The clinical treatment effect,lung function,inflammatory response and incidence rates in the two groups were observed.Results The treatment total effective rate in the study group was 91.43%,higher than that of 71.43%in the control group(P〈0.05).The levels of FEV1,FVC,PEF and PEF25 in the two groups after treatment were significantly increased compared with those before treatment(P〈0.05),which in the study group were significantly increased compared with those in the control group(P〈0.05).The levels of IL-6,IL-8,IL-13 and TNF-αin the two groups after treatment were significantly decreased compared with those before treatment(P〈0.05),which in the study group were significantly decreased compared with those in the control group(P〈0.05).The incidence rate of adverse reactions in the study group was 14.29%,lower than that of 17.14%in the control group(P〉0.05).Conclusion The application of compound ipratropium bromide combined with budesonide on lung function and inflammatory response in children with acute attack of bronchial asthma can effectively improve the lung function and inflammatory response and increase the treatment effect.
