主观全面评价法与微型营养评价法在食管癌患者中的应用 被引量:8

Application of subjective global assessment and mini-nutritional assessment in the esophageal cancer

摘要 目的研究主观全面评价法(SGA)和微型营养评价法(MNA)在食管癌患者中的应用价值。方法选取2014年12月-2015年12月河北北方学院附属第一医院胸外科收治的60例食管癌患者,分别应用SGA和MNA进行营养评估,并分析其与单项营养评价指标的关系,从而筛选出适合食管癌患者的最佳营养评价方法。结果SGA评价显示,食管癌患者的营养不良发生率为30.00%(18/60),MNA评价提示,食管癌患者发生营养小良的概率为45.00%(27/60),一致性检验结果显示,差异有统计学意义(X^2=18.51,P=0.01)。依据SGA评定分级,营养良好和轻-重度营养不良的体重指数(BMI)、肱三头肌皮褶厚度(TSF)、上臂肌围(AMC)、视黄醛结合蛋白(RBP)、前清蛋白(PAB)、清蛋白(ALB)、血红蛋白(H1))、氮平衡(NB)等比较,差异有统计学意义(P〈0.05),而淋巴细胞计数(TLC)差异无统计学意义(P〉0.05)。依据MNA评定分级,营养良好和轻-重度营养不良的BMI、AMC、RBP、Hb、TLC、NB等比较,差异有统计学意义(P〈0.05),而TSF、PAB和ALB差异无统计学意义(P〉0.05)。结论SGA和MNA在食管癌患者营养状况评估方面均具有一定的应用价值,但与传统单项营养评价指标的关系方面,SGA更优于MNA. Objective To discuss the application value of subjective global assessment (SGA) and mini-nutritional as- sessment (MNA) in patients with esophageal carcinoma. Methods 60 patients with esophageal carcinoma in the First Affiliated Hospital of Hebei North University from December 2014 to December 2015 were selected and the nutritional status were measured by SGA and MNA. The relationship between individual nutrition indicators and SGA, MNA was analyzed in order to select the best nutritional evaluation method flu patients with esophageal cancer. Results The results of SGA and MNA showed the incidence rate of malnutrition was 30.00% (18/60) and 45.00% (27/60) respec- tively. The consistency test resuhs showed that the difference was statistical significantly between SGA and MNA (X^2= 18.51, P=0.01). According to SGA grading, there were statistical differences in body mass index (BMI), triceps skin fold (TSF), arm muscle cirumference (AMC), retinol binding protein (RBP), prealbumin (PAB),albumin(ALB), hemoglobin (Hb) and nitrogen balance (NB) between good nutrition and light-severe malnutrition (P 〈 0.05), there was no differ- ence in total lymphocyte count (TLC) between good nutrition and light-severe malnutrition (P 〉 0.05). According to MNA grading, there were statistical differences in BMI, AMC, RBP, Hb, TLC and NB between good nutrition and light- severe malnutrition (P 〈 0.05), and there was no differences in TSF, PAB and ALB between good nutrition and light- severe malnutrition (P 〉 0.05). Conclusion SGA and MNA in the nutritional evaluation of patients with esophageal can- cer is of some application value, but in relevance to the traditional single nutrition indicators, SGA is more superior to MNA.
