摘要
目的探讨妊娠期行宫腔操作与宫腔粘连程度的关系。方法回顾性分析2014年2月至2015年10月重庆市妇幼保健院收治的宫腔粘连患者共92例,根据其末次妊娠的宫腔操作方式分为人流组(37例)、稽留流产组(46例)及产后清宫组(9例)。比较三组患者宫腔粘连程度及术后月经改善情况,并分析宫腔操作次数与宫腔粘连程度及宫腔粘连分离术后月经改善情况的关系。结果产后清宫组宫腔粘连轻度发生率及月经改善率最低,其次是稽留流产组,人流组最高,但三组患者宫腔粘连情况及月经改善情况比较,差异均无统计学意义(P>0.05);宫腔操作次数越多,宫腔粘连轻度发生率及月经改善率越低,但不同操作次数宫腔粘连情况及月经改善情况比较,差异均无统计学意义(P>0.05)。结论宫腔粘连的严重程度与宫腔操作次数及末次妊娠的宫腔操作方式无明显相关性,故清宫术中医生宫腔操作技术及术后辅助治疗在清宫术中尤为重要。
Objective To study the correlation between uterine cavity operation and intrauterine adhesion in pregnant stage. Methods Ninety-two patients with intrauterine adhesion in our hospital from February 2014 to October 2015 were retro spectively analyzed. The cases were divided into the artificial abortion group（37 cases）,missed abortion group（46 cases） and post partum curettage group（9 cases） according to the uterine cavity operation mode at the last pregnancy. The intrauterine adhesion degree and postoperative menstruation improvement were compared among 3 groups. And the relation between the intrauterine operation times with intrauterine adhesion and menstruation improvement after adhesion separation operation was analyzed. Results The incidence rate of mild intrauterine adhesion and menstrual improvement rate in the postpartum curettage group were the lowest,followed by the missed abortion group,which in the artificial abortion was the highest,but the differences were not statistically significant（P〉0.05）;the more the intrauterine operation times,the lower the incidence rate of mild intrauterine adhesion and menstrual improvement rate,but the difference was not statistically significant（P〉0.05）. Conclusion There is no correlation between the intrauterine adhesion severity with the intrauterine operation times and intrauterine operation at the last pregnancy,therefore the intrauterine operation technique in curettage and postoperative auxiliary therapy are especially important in curettage.
