摘要
目的:观察补肾利湿法对痛风性关节炎关节腔滑膜组织尿酸盐阴离子转运蛋白1(URAT1)的影响,探讨其降血尿酸及防治痛风性关节炎骨损伤的机制。方法:将84只SD大鼠随机分为空白组、模型组、中药对照组、西药对照组及补肾利湿法处方低、中、高剂量组,每组12只;予相应灌胃干预7 d后,除空白组外均采用尿酸钠联合氧嗪酸钾诱导大鼠急性痛风性关节炎模型。采用HE染色观察滑膜组织病理变化;紫外分光光度计检测大鼠血尿酸水平;Western blot法及免疫组化法检测滑膜组织中URAT1的表达情况。结果:滑膜组织切片HE染色结果显示,空白组大鼠关节滑膜细胞形态规则,排列整齐;模型组细胞排列杂乱,部分坏死脱落,组织水肿,间质疏松;西药及中药治疗组部分滑膜细胞边缘略不规整,组织结构基本完整。血尿酸水平,模型组较空白组血尿酸水平明显增高(P<0.05),各治疗组血尿酸较模型组明显下降(P<0.05)。免疫组化结果显示,模型组滑膜组织中URAT1阳性细胞明显增多,西药对照组、补肾利湿方中剂量组和高剂量组URAT1阳性细胞数较模型组明显减少(P<0.05)。Western blot结果显示,模型组URAT1蛋白表达水平较空白组明显增高,差异具有统计学意义(P<0.05);与模型组比较,中药对照组、西药对照组、补肾利湿方高剂量组URAT1蛋白的表达水平明显减少,差异有统计学意义(P<0.05)。结论:补肾利湿法可能通过下调痛风性关节炎关节腔滑膜组织URAT1的蛋白表达降低血尿酸水平,从而防治痛风性关节炎骨损伤。
Objective: To observe the effects of tonifying kidney and resolving dampness method on urate anion transporters 1( URAT1) in synovial tissue of articular cavity in gouty arthritis( GA),and discuss its mechanism on decreasing serum uric acid and prevention and treatment of bone injury in GA. Methods: Eighty four SD rats were randomly divided into the blank group,model group,Chinese medicine group,western medicine group,prescriptions groups of tonifying kidney and resolving dampness method with low-,mid-and high dose,twelve rats in each group. After oral administration with the corresponding drugs for 7 day,the uric acid sodium and oxonic acid potassium were used to induce acute GA model in rats except the blank group. The pathological changes of synovial tissue were observed by HE staining. The level of blood uric acid in rats was detected by ultraviolet photometer. The expression of URAT1 in synovial tissue was determined by Western blot and immunohistochemistry. Results: The HE staining results of synovial tissue sections showed that the articular synovial cells of rats in the blank group were normal in morphology and arranged neatly,while the cells in the model group arranged in disorder with the appearance of partial necrosis and fall off,tissue edema and loose mesenchyme. The synovial cells of rats treated with Chinese medicine and western medicine were partly and slightly irregular on the edge,but the structure was basically complete.Compared with the blank control group,the level of blood uric acid in the model groups was obviously increased( P〈0. 05).Compared with the model group,the blood uric acid in all the treatment groups were significantly decreased( P〈0. 05). The immunohistochemistry results showed that URAT1 positive cells of synovial tissue in the model group were obviously increased.Compared with the model group,URAT1 positive cells in the western medicine group and prescriptions groups of tonifying kidney and resolving dampness method with mid-and high dose were obvious
