摘要
目的:研究复融周期中囊胚移植后的妊娠结局与冷冻前的囊胚质量、发育天数的相关性,探讨单囊胚移植在复融周期中的可行性。方法回顾性分析2013年1月至2016年3月行冻融囊胚移植(FET)助孕患者的临床资料。入组患者均为第一周期第一次复融且行囊胚移植(n =1310)。比较囊胚移植后的胚胎着床率、临床妊娠率、异位妊娠率、早期流产率等临床结局指标。结果冷冻前 D5囊胚的优质囊胚率显著高于 D6囊胚(P〈0.05)。D5冻融囊胚移植后的胚胎着床率及临床妊娠率显著高于 D6冻融囊胚(P〈0.05),而早期流产率和异位妊娠率比较差异未见统计学意义(P〉0.05)。对于单囊胚移植的患者而言,冻融移植 D5/ D6优质囊胚的妊娠结局各项指标间比较差异未见统计学意义(P〉0.05)。若患者移植2枚优质囊胚,其胚胎着床率及临床妊娠率显著高于移植2枚非优质囊胚者(P〈0.05),而流产率和异位妊娠率比较差异未见统计学意义(P〉0.05)。结论 D5冻融囊胚移植后的胚胎着床率及临床妊娠率优于 D6囊胚。冻融移植单枚 D5/ D6优质囊胚的妊娠结局无差别。冻融移植2枚囊胚者较移植1枚及以上优质囊胚者妊娠结局较好。
Objective To analyze the correlation of the clinical outcomes among Day 5 and Day 6 blastocysts with different quality score prior to frozen-thawed embryos transfer aimed at studying the possibility of single blastocyst transfer. Methods In the retrospective study,the clinical data of patients underwent the first FET cycles in Reproductive Medicine Institute of Henan Province People's Hospital from January 2013 to March 2016 were retrospectively analyzed. The clinical outcomes were compared. Results The high quality blastocyst rate of day 5 was significantly higher than that of day 6(P〈 0. 05). Moreover,the implantation rate and pregnancy rate of day 5 frozen-thawed blastocysts transfer was significantly higher than that of day 6(P〈 0. 05),while there was no significant difference in the early abortion rate or ectopic pregnancy rate(P〉 0. 05). For the single high quality blastocyst transfer,there was no significant difference in the pregnancy outcome of day 5 and day 6 blastocyst(P〉 0. 05). If two high quality blastocyst were transferred,the implantation rate and pregnancy rate was significantly higher than that of transferred two low quality blastocyst( P〈 0. 05). However,the early abortion rates and ectopic pregnancy rates of the two groups had no significant difference. Conclusions The implantation rate and pregnancy rate of day 5 frozen-thawed blastocysts transfer is significantly higher than that of day 6. For the single high quality blastocyst ransfer,the pregnancy outcome of day 5 and day 6 blastocyst had no significance,while for the two blastocysts transfer,the pregnancy outcome of one or two high quality blastocyst transfer is better than that of low quality blastocyst transfer.
