摘要
目的比较不同时期冻融囊胚移植后的妊娠结局。方法回顾性分析2012年8月—2014年8月在沈阳东方医疗集团菁华医院生殖中心进行冻融囊胚移植的3 952个周期,从<35岁和≥35岁2个年龄段分别比较Day 5与Day 6单囊胚组、Day 5与Day 6双囊胚组移植后的临床妊娠率、囊胚种植率、流产率、异位妊娠率、多胎妊娠率等指标。结果在2个年龄段中,无论Day 5组还是Day 6组,双囊胚移植与单囊胚移植相比,临床妊娠率高,囊胚种植率低,多胎妊娠率高且差异极显著;同年龄段比较,流产率差异无统计学意义;在年龄≥35岁患者中,Day 5双囊胚组的异位妊娠率明显高于其他3组,差异显著。结论患者无论年龄大小,可以选择性单囊胚移植;Day 5期囊胚和Day 6期囊胚复苏移植后都有较好的妊娠结局,但Day 5期优于Day 6期,建议复苏时优先选择Day 5期囊胚。
[Objective] 1To compare the pregnancy outcome of frozen-thawed blastocysts transfer at different points in time. [ Methods ] 3 952 cycles of frozen-thawed blastocyst transfer in the Reproductive Center of Jinghua Hospital of Shenyang Eastern Medical Group in August 2012 to August 2014 were retrospectively analyzed, and they were divided into four groups according to two age groups （less than 35 years old and over or equal to 35 years old）： single blastocyst transferred on Day5, single blastocyst transferred on Day6, double blastocysts transferred on Day5, and double blastocysts transferred on Day6. The clinical pregnancy rate, implantation rate, abortion rate, ectopic pregnancy rate, and multiple pregnancy rate were compared. [Results ] In two age groups, the clinical pregnancy rate and multiple pregnancy rate in double blastocysts transfer group were higher than those in single blastocysts transfer group, while the implantation rate was lower than that in single blastocysts transfer group, the differences were statistically significant, and the results were observed in both Day5 group and Day6 group. There was no statistically significant difference in abortion rate between two age groups. In over or equal to 35 years old group, the ectopic pregnancy rate of double blastocysts transferred on Day5 group was higher than that in other three groups, and the differences was statistically significant. [Conclusion] Regardless of age, patients can choose single blastocyst transfer. The pregnancy outcomes in frozen-thawed blastocysts transfer of Day5 and Day6 are satisfactory, while the former＇s effect is better than that of the later, and it is suggested that patients can choose Day5 frozen-thawed blastocysts transfer.
