摘要
目的探讨黄体生成素(LH)峰值日血清高孕酮对自然周期冻融囊胚复苏移植(FBT)助孕周期临床妊娠结局的影响。方法收集行冻融囊胚移植助孕治疗的522个周期,按LH峰值日血清孕酮值分为正常孕酮组(P≤1.5 ng/ml)和高孕酮组(P>1.5 ng/ml),并对影响自然周期冻融囊胚移植妊娠结局的各因素进行Logistic回归分析。结果患者的年龄、不孕年限、基础卵泡刺激素(FSH)、体重指数(BMI)、内膜厚度、移植囊胚数、移植周期数、早期流产率、异位妊娠率、多胎率组间均无统计学差异(P>0.05)。正常孕酮组的囊胚种植率、临床妊娠率分别为46.05%(332/721)、61.50%(246/400),显著高于高孕酮组的37.56%(83/221)、50.82%(62/122),差异有统计学意义(P<0.05)。另外,Logistic回归分析显示,子宫内膜厚度、LH峰值日血清孕酮值升高可影响妊娠结局。结论 LH峰值日血清孕酮水平升高引起自然周期冻融囊胚移植妊娠率明显下降。
Objective: To explore the influence of progesterone (P) elevation on clinical outcomes of frozen-thawed blastocyst transfer cycles performed in natural cycles (FBT-NC) . Methods: A retrospective analysis was performed on 522 blastocyst thawing cycles. The cycles were divided into two groups according to the serum progesterone levels on the day of luteinizing hormone (LH) surge: normal P level (P≤1.5 ng/ml) and P elevation group (P〉1.5 ng/ml) . Basic characteristics and pregnancy outcomes between the two groups were compared. In addition, the Logistic regression for the relationship between clinical pregnancy and the related factors was done. Results: There were no statistical differences between the two groups with regard to the age of the women, duration of infertility, basal FSH (follicle-stimulating hormone) , BMI (body-mass index) , endometrial thickness, number of embryos transferred, early abortion rate, ectopic pregnancy rate and multiple pregnancy rate (P 〉 0.05) , however, patients in normal P level group achieved a significantly higher implantation rate [46.05% (332/721) vs 37.56% (83/221) ] and pregnancy rate [61.50% (246/400) vs 50.82% (62/122) ] (P 〈 0.05) . Furthermore, the Logistic regression showed that the endometrial thickness and serum progesterone levels were significantly associated with clinical pregnancy rate in FBT-NC. Conclusion: Progesterone elevation on the day of LH surge may impair the clinical pregnancy rate of FBT-NC.
