摘要
目的:探讨稳心颗粒联合阿替洛尔对老年气滞血瘀型冠心病患者心率变异性的影响,并研究联合用药在冠心病螺旋CT血管造影(CTA)诊断中的应用价值。方法:选取122例老年气滞血瘀型冠心病患者,采用随机数表法将其分为观察组与对照组,每组61例。观察组患者口服稳心颗粒2周,检查的当日联合应用阿替洛尔后进行CT扫描;对照组检查当日口服阿替洛尔后进行CT扫描。观察比较两组患者心率、心率波动控制情况,分析心率及重建时相对CTA图像质量的影响。结果:观察组患者治疗后冠脉成像质量右冠脉(RCA)、左前降支(LAD)、左旋支(LCX)评分及综合评分明显优于对照组,两组相比有差异(t=0.870,t=0.903,t=1.097,t=0.913;P<0.05)。观察组检查过程中平均心率、心率波动及率变异性(HRV)明显优于对照组,两组相比有差异(x^2=6.785,x^2=6.844,x^2=12.151;P<0.05)。结论:稳心颗粒联合阿替洛尔治疗可有效改善气滞血瘀型冠心病患者心律失常及心率变异性,提高冠状动脉CTA成像质量,具有临床应用及推广价值。
Objective： To investigate the effect of Wenxin granules and atenolol on heart rate variability（HRV） in elderly patients with coronary heart disease, and to study the value of combination therapy in the diagnosis of spiral CT angiography（CTA） of coronary artery disease. Methods： 122 cases of elderly patients with coronary heart disease were selected with qi stagnation and blood stasis, and all the patients were randomly divided into observation group and control group, 61 cases in each group. The patients in the observation group were given Wenxin granules for 2 weeks. CT was performed on the same day after the application of atenolol. The patients in the control group were treated with atenolol for CT scan. Compared with the heart rate and heart rate fluctuation between the two groups, we analyzed the effects of heart rate and CTA image quality. Results： The left anterior descending coronary artery（LAD）, left anterior descending coronary artery（LCX） score and comprehensive score were significantly better than those of the control group（t=0.870, t=0.903, t=1.097, t=0.913; P〈0.05）. In the observation group, the mean heart rate, heart rate fluctuation and HRV were significantly higher than that in the control group. The differences were statistically significant（x^2=6.785, x^2=6.844, x^2=12.151; P〈0.05）. Conclusion： Wenxin granules combined with atenolol can effectively improves the arrhythmia and heart rate variability in patients with coronary heart disease with qi stagnation and blood stasis, and improves the quality of coronary artery CTA, which has clinical application and popularization value.
