个体化碘海醇注射方案对冠状动脉CT血管造影强化程度及成像质量的影响 被引量:1

摘要 目的探讨个体化碘海醇注射方案对冠状动脉CT血管造影强化程度及成像质量的影响。方法选取2016年6月至2017年12月本院收治的疑似冠心病患者186例,采用随机数表法将其分为对照组、A组及B组,每组各62例。对照组患者固定对比剂注射速度,A组患者基于体表面积调节对比剂注射速度,B组患者基于体重调节对比剂注射速度。所有患者均行冠状动脉CT血管造影检查,分析三组患者冠状动脉强化程度、成像质量、对比剂剂量、对比剂流速、有效辐射剂量及剂量长度乘积。结果三组患者冠状动脉强化值、冠状动脉CT血管造影成像质量、对比剂剂量和流速比较差异均有统计学意义(F = 6.891、6.344、96.901、177.773,均P < 0.05);A组患者冠状动脉强化值显著高于对照组和B组(均P < 0.05);对照组患者冠状动脉CT血管造影成像质量评分显著低于A组和B组(均P< 0.05);A组患者对比剂剂量显著低于对照组和B组(均P< 0.05);对照组患者对比剂流速显著高于A组和B组(均P < 0.05);三组患者有效辐射剂量、剂量长度乘积比较差异均无统计学意义(F = 2.003、0.022,P = 0.138、0.983)。结论基于体表面积制订个体化对比剂注射方案,可有效提高冠状动脉CT血管造影强化程度及成像质量,值得临床推广使用。 Objective To investigate the effect of individual Iohexol injection on the intensity and imaging quality of coronary CT angiography. Method 186 patients with suspected coronary artery heart disease admitted to our hospital from June 2016 to December2017 were selected and divided into control group, group A and group B by random number table method, with 62 cases in each group. Control group patients fixed the injection rate of contrast agent, group A patients adjusted the injection rate of contrast agent based on body surface area, and group B patients adjusted the injection rate of contrast agent based on body weight. Coronary artery CT angiography was performed in all patients, and coronary artery enhancement, imaging quality, contrast agent dose, contrast agent flow rate and effective radiation dose were analyzed among the three groups. Result There were statistically significant differences in coronary artery enhancement value, coronary artery CT angiography imaging quality, contrast agent dose and flow velocity among the three groups(F=6.891, 6.344, 96.901, 177.773, all P<0.05). Coronary artery enhancement values in group A were significantly higher than those in control group and group B(all P<0.05);the quality of coronary angiography in control group was significantly lower than those in group A and group B(all P<0.05);the dose of contrast agent in group A was significantly lower than those in control group and group B(all P<0.05);the contrast agent flow rate was significantly higher in control group than that in group A and group B(all P<0.05). There were no significant differences in effective radiation dose and dose length product among the three groups(F=2.003, 0.022, P=0.138, 0.983). Conclusion The individualized contrast agent injection scheme based on body surface area can effectively improve the intensity and imaging quality of coronary CT angiography, which is worthy of clinical application.
