摘要
目的:探讨肉苁蓉对气管切开插管大鼠肺部病理及肺泡灌洗液(BALF)肺泡表面活性物质相关蛋白A(SPA)的影响。方法:将54只SD雄性大鼠采用随机数字表法分为A组(正常组18只)、B组(气管切开插管模型组18只)、C组(气管切开插管+肉苁蓉干预18只)。各组分别于24 h、72 h、168 h 3个时段取材6只。取材时结扎并取右肺组织行病理切片检查,采用Smith评分半定量分析;于左肺取肺泡灌洗液,应用SP-A ELISA试剂盒检测SP-A浓度。结果:肺组织病理切片显示B组、C组大鼠24 h、72 h、168 h时段均有不同程度的水肿、肺泡及间质炎症、出血、肺不张等肺组织损伤,Smith评分高于对应时段正常大鼠。C组3个时段评分与B组对应时段评分比较无明显差异。气管切开插管大鼠24 h、72 h、168 h 3个时段肺组织损伤程度不同,随着时间增加损伤程度不断加重,3个时段有明显差异。B组、C组大鼠肺泡灌洗液SP-A浓度在24 h、72 h、168 h 3个时段均明显高于对应时段空白对照组,以24 h浓度最高,168 h最低,呈随气管切开插管时间的延长而浓度不断降低的趋势,C组168h SP-A浓度高于B组。结论:气管切开插管可导致水肿、肺泡炎症、出血、肺不张等肺组织损伤及肺部固有免疫功能紊乱,SP-A有保护肺组织作用,早期SP-A代偿性增高,随后逐渐下降,而肉苁蓉在后期可提高SP-A的浓度而抑制肺组织的损害。
Objective:To observe the effects of Herba Cistanches on lung tissue pathology and SP - A in BALF in rats with tracheostomy tube. Methods:All of 54 male SD rats were randomly divided into 3 groups:normal control( A), model with tracheostomy tube(B) ,the herba cistanches - treatment(C). Each 6 rats of each group were killed after 24 h ,72 h, 168 h and the lung tissues were sent to take pathological examination immediately. Smith scores were used to evaluate the injuries. The concentrations of SP - A in BALF were detected with ELISA method. Results : The observation of lung tissue pathology was found normal in group A and injuries such as edema, alveolar and interstitial inflammation, alveolar and interstitial hemorrhage, atelectasis were observed in group B and C. The Smith scores in group B and C were higher than group A. The injuries of lung tissues examined at three phases (24 h,72 h,168 h) were different in group B and C. They became severer as the time went on. The concentrations of SP - A in BALF in group B and C were significantly higher than group A at 24 h,72 h and 168 h and highest at 24 h,lowest at 168 h,they became more and more lower as the time went on. Conclusion:The tracheostomy tube on rats may lead to lung injuries, such as edema, alveolar and interstitial inflammation, alveolar and interstitial hemorrhage, atelectasis and also lead to immunologic derangement. The SP - A shows its protective function and its level in BALF increases first and then gradually decreases. The herba cistanches can protect lung tissue through improving the concentration of SP - A.
