摘要
目的探讨肝炎后肝硬化与酒精性肝硬化的临床表现与实验室检查指标差异。方法 选择2013年9月～2015年9月我院接受治疗的肝炎后肝硬化确诊患者20例作为肝炎组,同期治疗的酒精性肝硬化确诊患者20例作为酒精组。采用回顾性研究方法,研究两组患者的临床表现和并发症发生情况。结果 酒精组的右上腹疼痛、黄疸、肝大发生率高于肝炎组,腹水、脾大发生率低于肝炎组;酒精组的AST/ALT、谷氨酰转肽酶、三酰甘油、胆固醇、血清尿酸值均高于肝炎组;酒精组的自发性腹膜炎发生率明显高于肝炎组,肝炎组的肝肾综合征、原发性肝癌、肝性脑病的发生率显著高于酒精组,以上差异均有统计学意义（P〈0.05）。结论 肝炎后肝硬化与酒精性肝硬化具有不同的临床表现和特点,可为临床鉴别两者提供参考依据。
Objective To investigate the difference of clinical manifestation and laboratory examination index between posthepatitic cirrhosis and alcoholic cirrhosis.Methods 20 definite cases with posthepatitic cirrhosis received and treated by our hospital from September 2013 to September 2015 were selected as hepatitis group,and 20 definite cases with alcoholic cirrhosis for treatment at the same period were selected as alcoholic group.The clinical manifestations and complications of the two groups were analyzed using retrospective study method.Results The incidence rate of right upper quadrant abdominal pain,jaundice and hepatomegaly in alcoholic group was higher than that of posthepatitic cirrhosis group,and incidence rate of ascites and splenomegaly was lower than that of posthepatitic cirrhosis group;AST/ALT,valley aminoacyl transfer peptidase,triglyceride glycerol,cholesterol and serum uric acid value of alcoholic group was higher than that of hepatitis group;the incidence rate of spontaneous bacterial peritonitis in alcohol cirrhosis group was significantly higher than that in hepatitis group,while the incidence rate of liver and kidney syndrome,primary liver cancer and hepatic encephalopathy in hepatitis group was significantly higher than that in alcoholic group respectively.All the above-mentioned difference was statistically significant（P〈0.05）.Conclusion There are different clinical manifestation and characteristic in posthepatitic cirrhosis and alcoholic cirrhosis which provide reference for clinical differentiation.
