摘要
目的:探讨不同剂量糖皮质激素在缓解带状疱疹患者神经疼痛中的疗效。方法:84例带状疱疹伴神经疼痛患者采用随机数表法分为观察组和对照组,各42例,均给予伐昔洛韦与普瑞巴林行正规抗病毒治疗,同时对照组给予小剂量(20mg/d)泼尼松,观察组给予中等剂量(40mg/d)泼尼松,均治疗2周,比较两组止泡时间、水泡消失时间、结痂时间、脱痂时间,同时应用简化Mc Gill疼痛量表评价两组治疗前及治疗后1周、治疗后2周疼痛度,并观察不良反应。结果:观察组止泡时间、水泡消失时间、结痂时间、脱痂时间明显短于对照组(P<0.05);观察组治疗后1周、治疗后2周Mc Gill疼痛评分均低于对照组及治疗前(P<0.05);两组不良反应发生率比较无显著差异(P>0.05)。结论:应用中等剂量糖皮质激素缓解带状疱疹病毒患者神经疼痛效果较好,可减少止泡时间、水泡消失时间、结痂时间、脱痂时间,不良反应轻。
Objective： To investigate the effect of different doses of glucocorticoids in the treatment of pain in patients with herpes zoster. Methods： 84 cases of herpes zoster patients with neuropathic pain were randomly divided into observation group and control group, 42 cases in each, were treated with valaciclovir and pregabalin for regular antiviral therapy, while the control group was given small dose of prednisone（20mg/d）, the observation group was given moderate doses of prednisone（40mg/d）, were treated for 2 weeks, compared with two groups of check global time, bulla disappeared time, the time of incrustation and decrustation, and application of the simplified Mc Gill pain questionnaire evaluation of two groups before and after treatment for 1 weeks, 2 weeks after the treatment of pain, and observe the adverse reaction. Results： The observation group for global time, blisters disappearing time, scab, decrustation time was shorter than the control group（P〈0.05）; the observation group after treatment for 1 weeks and 2 weeks after treatment Mc Gill pain scores were lower than the control group and before treatment（P〈0.05）; adverse reactions of two groups were no significant difference（P〈0.05）. Conclusion：The application of medium dose glucocorticoid relief effect of herpes zoster virus in patients with nerve pain better, can reduce the bubble stopping time, bubble disappearance time and the time of incrustation and decrustation, light adverse reactions.
