摘要
目的观察补肾断续汤治疗绝经后骨质疏松症的疗效及对血清基质金属蛋白酶-2(MMP-2)和骨代谢相关指标水平的影响。方法将106例绝经后骨质疏松症患者随机分为观察组53例及对照组53例,对照组给予钙尔奇D加骨化三醇治疗,观察组在对照组治疗基础上给予补肾断续汤治疗,2组均以1个月为1个疗程,均连续治疗3个疗程。观察2组治疗前后疼痛评分、骨密度(BMD)、血清MMP-2水平及骨代谢指标骨特异性碱性磷酸酶(B-ALP)、骨钙素(OC)、抗酒石酸酸性磷酸酶(TRACP)水平变化情况,统计2组临床疗效。结果 2组治疗后疼痛程度评分均明显降低(P均<0.05),且观察组疼痛程度评分明显低于对照组(P<0.05)。对照组治疗后腰椎正位、Wards三角区、大粗隆BMD水平及血清MMP-2、B-ALP、OC、TRACP水平与治疗前比较差异均无统计学意义(P均>0.05);观察组治疗后腰椎正位、Wards三角区、大粗隆BMD水平及血清B-ALP和OC水平均显著高于治疗前及对照组(P均<0.05),血清MMP-2和TRACP水平均显著低于治疗前及对照组(P均<0.05)。观察组总有效率明显高于对照组(P<0.05)。结论补肾断续汤能够明显缓解PMOP患者的疼痛程度,提高骨密度,抑制破骨细胞活动的同时促进成骨细胞的增殖,值得临床推广应用。
Objective It is to observe the effect of Bushen Duanxu decoction on postmenopausal osteoporosis and its effect on the level of serum matrix metalloproteinase -2 （MMP -2） and bone metabolism indexes. Methods 106 cases of postmeno- pausal osteoporosis patients were randomly divided into observation group （n = 53 cases） and control group （n = 53 cases）, the control group was given Caltrate D plus ossification of calcitriol, the observation group was given Bushen Duanxu decoction on the basis of treatment in control group. Both groups were treated for 3 months. The changes of pain score, bone mineral density （BMD）, serum MMP- 2 and bone metabolism indexes such as B -ALP, OC and TRACP were observed before and after treatment and the curative effects were compared in both groups. Results After treatment, pain scores were significantly decreased in both groups, and the decrease was more significant in observation group. There was no significant difference in lumbar spine, Wards triangle and greater trochanter BMD in control group before and after treatment（ P 〉 O. 05 ）. In the obser- vation group after treatment, lumbar spine, Wards triangle and greater trochanter BMD levels, serum B -ALP and OC levels were significantly higher while serum MMP - 2 and TRACP levels were significantly lower than that before treatment and in control group after treatment（ P 〈 0. 05 ）. The total efficiency of the observation group was significantly higher than that in the control group. Conclusion Bushen Duanxu decoction can significantly improve the pain of PMOP patients, improve bone density, inhibit ostcoclast bone resorption activity, promote the proliferation of ostcoblasts, it is worthy of clinical application.
