摘要
颈椎病以"筋"和"骨"的病理改变为主,二者可相互影响。古恩鹏教授针刺治疗颈椎病的主穴有:百会、玉枕、天柱、颈夹脊、肩外俞、阿是穴、曲池。另根据患者症状辨证配穴。古恩鹏教授取穴精准但不追求得气,每针灸前必嘱患者安神定志,取舒适体位,进针前必探指下虚实,查其气血经脉循行,经气汇聚之所,或筋结之处,选穴有的放矢;进针伴随捻转手法,迅捷轻柔,患者无知觉而针已至其所,最大限度避免患者因紧张不适造成的抵抗。进针后针尖朝向痛处行针,以传达经气至病位,增强疗效,可与清插重提行针手法以提升气血。古师行针灸治疗注重医患配合、整体与局部兼顾等,并不断随患者反馈及经验积累改良治疗方案,还提倡筋骨并重、动静相合。此外,还常采用拔罐法、颈部保护、综合疗法辅助治疗。
Cervical spondylosis mainly appears the pathological changes of ＂muscle＂ and ＂bone＂ which can interact with each other. The major points Professor Gu En-peng uses for treating cervical spondylosis include Baihui （ GV 20）, Yuzhen （ BL 9 ） ,Tianzhu BL 10） ,neck Jiaji （Ex-B2）,Jianwalshu （SI 14）, Ashi,Quchi （LI 11 ）. Besides, proper points are matched according to the pa- tients＇ exact symptonrs. Professor Gu En-peng selects points accurately but not simply pursue arrival of qi. Each time before acupunc- ture he will enjoin patient to try to keep calm and take comfortable position. He always explores the actual situation before entering the needle .and checks qi and blood circulation in the meridians,the place where qi and meridians converge ,or the place where there are tendon knots. In a word ,he has a definite object in view when selecting points. While entering the needle,accompanying with rotating techniques,he makes the needle arrive the proper point quickly and gently while the patients don＇t realize it,which avoids patient＇s resistance to the maximum due to tension and discomfort. After the needle enters the body,the needle tip is towards the sore area to convey qi to the diseased area to enhance the efficacy. He uses inserting and lifting techniques to activate qi and blood. He also pays attention to cooperation between the doctor and the patient,both global and local, and constantly regulates treatment programs with the patient＇s feedback and his own experience accumulation. He advocates emphasizing both bones and tendons .static and dynamic consistency. In addition, he also often uses cupping, neck protection, and comprehensive therapy.
