摘要
探讨Ⅱ型糖尿病(T2DM)患者空腹血清游离脂肪酸(FFAs)水平与其他糖、脂代谢参数间的相关性。方法收集2015年6月至10月在南京医科大学第一附属医院就诊的113例新近诊断Ⅱ型糖尿病患者、55例糖尿病前期者,同时选取62例健康体检者。根据甘油三酯(TG)是否〉1.72mol/L以及总胆固醇(TC)是否〉5.7mol/L分为血脂正常组,混合高脂血症组,高TC、TG组。测定其空腹血清FFAs,空腹血糖(FPG),餐后两小时血糖(2hPG),糖化血红蛋白(HbAle)及糖化白蛋白(GA)水平及相关脂代谢参数,并进行统计学分析。将糖尿病组根据糖化血红蛋白(HbAlC)结果分为血糖控制满意组(〈7%),血糖控制一般组(7%~7.9%),血糖控制不理想组(8%~8.9%),血糖控制较差组(≥9%),比较各组间FFA水平。结果T2DM患者空腹FFAs水平为(806.4±352.8)mol/L高于糖尿病前期及健康体检组(P〈0.01);血糖控制较差组FFAs水平明显高于血糖控制较好组(P〈0.01);FFAs与FPG、2hPG、HbAlc、GA、TG等指标有正相关(r分别为0.394,0.4,0.31,0.227,0.43l,P〈0.01),与HDL—C呈负相关(r=-0.156,P〈0.05);血糖异常人群中,约49.7%的FFA水平并无明显升高(P〉0.05);而伴高脂血症者占67.9%,其中FFA升高者占74.1%(P〈0.01)。结论FFAs在调节糖、脂代谢方面有重要的意义,FFAs水平与T2DM血糖控制情况有一定相关性,可能是糖尿病与高脂血症之间的联系分子。
Objective To characterize the association of free fatty acids (FFAs) with glucose and lipid metabolic related parameters in type 2 diabetes. Methods We measured fasting serum FFAs, fasting blood-glucose (FPG), 2 hour postprandial blood glucose (2hPG), glycosylated hemoglobin Alc (HbAlc), glycated albumin (GA) and lipid metabolic parameters in 113 cases of type 2 diabetic patients, 55 case of pre-diabetes and 62 cases of healthy subjects. Results The FFAs in type 2 diabetic patients (806.45=352.8 mol/L) was significantly higher than that in other groups. The glucose would upgrade besides the FFAs (P〈0.01). There were positive relations between FFAs with FPG,2hPG, HbAlc, GA and TG(r=0. 394,0.4,0.31,0. 227, 0. 431,P〈0.01), and negative relation between FFAs with HDL-C(r=-0. 156, P〈0.05). We observed that almost 49.7% person had abnormal glycouse don't have high FFAs. Conclusions FFAs plays an important role in glucose and lipid metabolism, may link diabetes with hyperlipidemia.
