摘要
目的 探讨舒芬太尼对高龄腹腔镜手术患者血流动力学的影响,并与芬太尼进行对比,旨在为临床合理使用麻醉药物提供相关理论参考依据。方法 选择2015年1月-2016年1月60例于我院行腹腔镜手术的高龄患者作为观察对象,随机分为Ⅰ组(舒芬太尼麻醉)和Ⅱ组(芬太尼麻醉),每组30例,比较Ⅰ组与Ⅱ组在T0、T1、T2、T3时点SBP、DBP、HR水平的变化情况、并对两组的镇痛效果进行对比分析。结果 Ⅰ组与Ⅱ组T0时点SBP、DBP、HR水平比较,差异无统计学意义(P〉0.05)。Ⅰ组与Ⅱ组T1时点SBP、DBP、HR水平分别较T0、T2、T3显著降低,差异具有统计学意义(P〈0.05),且Ⅱ组T1时点SBP、DBP、HR水平分别显著低于Ⅰ组,差异具有统计学意义(P〈0.05)。Ⅰ组与Ⅱ组的疼痛效果比较,差异具有统计学意义(P〈0.05),其中Ⅰ组Ⅲ级比率显著低于Ⅱ组,差异具有统计学意义(3.3%vs 20.0%,P〈0.05)。结论 舒芬太尼对高龄腹腔镜手术患者血流动力学影响较芬太尼小,且镇痛效果好,具有较好的应用价值。
Objective To investigate the effect of sufentanil on hemodynamics in elderly patients undergoing laparo- scopic surgery, and compared with fentanyl, aim to provide the theoretical reference for the rational use of narcotic drugs in clinic. Methods From January 2015 to January 2016 ,60 cases of elderly patients in our hospital underwent laparoscopic surgery were randomly divided into group Ⅰ (sufentanil anesthesia) and group Ⅱ (fentanyl anesthesia), 30 cases in each group,the SBP,DBP and the HR level changes in T0, T1, T2,T3 time of group Ⅰ and group Ⅱ were compared, and the analgesic effect of the two groups were compared. Results There was no significant difference in HR, DBP and SBP levels between group Ⅰ and group Ⅱ at T0(P〉0.05). The SBP, DBP, HR levels at T1 were significantly lower at T0, T2 and T3 between group Ⅰ and group Ⅱ , the difference was significant(P〈0.05). The SBP, DBP and HR levels at T1 in group Ⅱ were significantly lower than group Ⅰ , the difference was significant (P〈0.05). The pain effect in group Ⅰ and group Ⅱ were compared, the difference was significant(P〈0.05), the grade m ratio Ⅰ of group Ⅰ was significantly lower than that of group Ⅱ , the difference was significant(3.3% vs 20.0%, P〈0.05). Conclusion The inpact of sufentanil on hemodynamics in patients with elderly laparoscopic operation dynamics effect is less than that of fentanyl, and the analgesic effect is good, has good application value.
