摘要
目的评价改良喉罩联合地佐辛、丙泊酚用于纤支镜检查的麻醉效果及安全性。方法行纤支镜检查的患者60例,随机分为改良喉罩组(A组)和表面麻醉组(B组)。A组静脉依次给予地佐辛0.1~0.15mg/kg,丙泊酚l^2 mg/kg后置入改良喉罩,丙泊酚4~6 mg/kg/h维持麻醉;B组采用2%利多卡因雾化吸入法表面麻醉。记录麻醉前、手术开始前、过声门时、达隆突时、进镜5 min时和检查结束的血压、心率、血氧饱和度;记录体动、呛咳、屏气等不良反应,是否需要中断检查。结果镜检中A组血流动力学平稳,B组血流动力学波动明显,与麻醉前比较,差异有统计学意义(P<0.05);麻醉后,两组血流动力学比较,差异具有统计学意义(P<0.05);A组不良反应明显低于B组,差异有统计学意义(P<0.05)。结论改良喉罩联合地佐辛、丙泊酚应用于纤支镜检查安全可行。
Objective To evaluate the modiifed laryngeal mask combined with dezocine and propofol for bronchoscopy with the anesthetic effect and safety.Methods 60 cases of patients were randomly divided into modified laryngeal mask group (group A) and surface anesthesia group (group B). Group A was given for 0.1 - 0.15 mg/kg dezocine and propofol l - 2 mg/kg after implantation modiifed laryngeal mask, propofol 4 - 6 mg/kg/h anesthesia; Group B with 2% lidocaine aerosol inhalation anesthesia. The end records began before anesthesia, before surgery, when the glottis bulge into the mirror for 5 minutes and check the blood pressure, heart rate, blood oxygen saturation; records of body movement, choking cough, breath and other adverse reactions, whether you need to check interrupt.Results A group blood microscopic dynamics was stable. The group B hemodynamics lfuctuates significantly, there were statistically significant differences compared with before anesthesia (P〈0.05). After anesthesia, there was statistically signiifcant differences about two groups (P〈0.05); Group A of adverse reactions was signiifcantly lower than in group B (P〈0.05).Conclusion Modified laryngeal mask combined with propofol used in dezocine, bronchoscopy is safe and feasible.
