摘要
目的 探讨应用Acumed腓骨髓内钉内固定治疗腓骨远端骨折的临床疗效。方法 笔者自2013-12—2014-12应用Acumed腓骨髓内钉内固定治疗25例腓骨远端骨折,19例采用闭合复位,6例采用切开复位,术后24 h内进行肌肉舒缩功能锻炼。结果 所有患者均获得随访8-13个月,平均11个月,术后外踝部切口均一期愈合,未发生感染、内固定物松动、断裂等并发症。腓骨骨折获得骨性愈合,踝关节活动范围：跖屈（35.6±8.5）°、背伸（17.4±5.3）°。末次随访疗效按Baird-Jackson踝关节评分评定为（95.96±3.31）分,优18例,良6例,可1例,优良率96%。结论Acumed腓骨髓内钉治疗腓骨远端骨折,可明显缩短手术时间,减少软组织的剥离,配合术后早期功能锻炼,早期恢复踝关节功能;同时腓骨髓内钉能很好地维持腓骨长度以及腓骨外翻角,临床疗效满意。
Objective To investigate the clinical results and significance of Acumed intramedullary nail for fixation of the fibula fractures. Methods Twenty five cases of the closed distal fibula fractures were treated by Acumed intramedullary nail between December 2013 and December 2014, 19 cases by closed reduction and internal fixation and 6 cases by open reduction and internal fixation. Muscle diastolic function exercises were performed within 24 hours after operation. The results and significance were assessed by Baird-Jackson score and X-ray. Results All cases were followed up. The mean followed- up was 11 months （8-13 months）. There were no postoperative complications. Bone healing of the fibula was confirmed by X- ray in all the cases. The ROM of ankle joint was： plantar flexion （35.6±8.5）°, Dorsal stretch （17.4±5.3）°. The functional scores were （95.96±3.31） for the Baird-Jackson score, 18 cases were excellent, 6 were good, 1 was fair. The excellent and good rate was 96%. Conclusion The operative time can be shortened remarkably and the dissection of soft tissue can be reduced in fixation of the fibula fractures with Acumed intramedullary nail. A good function results can be achieved if combined with early function exercises. The intramedullary nail can maintain a good length and of fibula valgus angle and satisfactory clinical results could be obtained.
