摘要
目的探讨他莫昔芬治疗乳腺癌患者导致卵巢囊肿的原因并了解超声监测在诊断和治疗中的重要性。方法选择81例明确诊断为乳腺癌的患者,将符合内分泌治疗且服用他莫昔芬药物的患者33例定义为药物组,条件不符合而没有服用他莫昔芬治疗的患者48例定义为对照组,并将两组患者分别细分为绝经前和绝经后。采用阴道超声、腹部彩超等复查子宫、卵巢情况,数据包含卵巢囊肿、子宫内膜病变、子宫肌瘤等,了解他莫昔芬对卵巢的作用及超声复查的重要性。结果在超声的严密监测下,药物组共发现12例患者发生卵巢囊肿,而对照组仅有4例,两组相比差异有统计学意义(P<0.05)。在同时服用他莫昔芬的情况下,绝经前患者更易发生卵巢囊肿;而在相同的绝经前或绝经后的情况下,服用他莫昔芬发生卵巢囊肿的概率明显高于未服用的患者,两者对比差异有统计学意义(P<0.05)。结论他莫昔芬可增加乳腺癌患者卵巢囊肿的发生概率,因此超声监测不仅对病情有一定帮助,且有利于治疗的干预,提高乳腺癌治愈率以及患者的满意度。
Objective To explore the causes of ovarian cysts induced by tamoxifen in patients with breast cancer,understand the importance of ultrasound monitoring in diagnosis and treatment. Methods Eighty-one patients diagnosed as breast cancer definitely were selected and divided into drug group and control group,33 patients in drug group were treated by tamoxifen,while 48 patients in control group were not treated by tamoxifen. The patiens in the two groups were further divided into premenopause subgroup and postmenopause subgroup.Transvaginal ultrasound and abdominal ultrasound were used to reexamine uterine and ovarian conditions,including ovarian cysts,endometrial lesions,and hysteromyoma. The effect of tamoxifen on ovary and the improtance of ultrasound were analyzed. Results There was statistically significant difference in the incidence rate of ovarian cysts between drug group（ 12 patients） and control group（ 4 patients）（ P〈 0. 05）. Under the same condition of taking tamoxifen,ovarian cysts easily occurred in premenopausal patients. Under the same condition of premenopause and postmenopause,the incidence rate of ovarian cysts in drug group was statistically significantly higher than that in control group（ P〈0. 05）. Conclusion Tamoxifen can increase the incidence rate of ovarian cysts in patients with breast cancer. Therefore,ultrasound is not only helpful to diagnose the disease,but also is beneficial to treatment of the disease,and ultrasound can improve the cure rate and satisfaction degree of the patients.
