摘要
目的探讨不同预防性抗菌药物方案在痔疮手术围手术期的成本-效果分析。方法收集我院2014年1月至2016年3月符合纳入标准的痔疮患者195例,随机分为4组:A组48例、B组48例、C组49例、D组50例,4组给予4种不同的预防性抗菌药物比较4组患者的不良反应发生率、术前术后外周血白细胞、切口感染方面及抗菌药物费用等指标。结果 4组患者在手术时间、术中出血量、住院时间、术前术后外周血白细胞、切口感染方面及不良反应发生率方面,组间无显著差异(P>0.05),而在抗菌药物费用方面,相比其他3组,D组患者费用明显更少,差异具有统计学意义(P<0.05)。结论选择短疗程应用头孢唑啉钠联合甲硝唑作为痔疮围手术期的预防性抗生素方案,抗生素药物费用低,不仅可达到预防术后感染的目的,也可减少围手术期抗生素的过度使用,值得临床进一步推广应用。
Objective To determine the cost-effectiveness analysis of different prophylactic antibiotics in the perioperative period of hemorrhoids surgery. Methods 195 cases of hemorrhoids were collected in our hospital from January 2014 to March 2016,who were randomly divided into 4 groups：48 cases of the A group,48 cases of the B group,49 cases of the C group,50 cases of the D group. The rate of adverse reactions,the number of peripheral blood leukocytes before and after the operation,the rate of incision infection and the cost of antibiotics among the four groups were compared. Results The length of operative time and hospital stays,the volume of blood loss,the rate of incision infection and adverse reaction,the number of peripheral blood leukocytes,among the four groups had no significant difference（P〈0.05）. However,compared with the other three groups,the cost of antibacterial drugs of the D group was significantly less（P〈0.05）. Conclusion Combination of cefazolin sodium salt and metronidazole in a short course was a better choice as the prophylactic antibiotics regimen in the perioperative period of hemorrhoids. since it was safe,effective and less costly. The regimen is worthwhile for prompting in clinical practice.
