慢性乙型肝炎核苷酸类似物耐药患者外周血IL-21、γ-IFN、IL-10及β-TGF变化的临床研究 被引量:5

摘要 目的探析慢性乙型肝炎核苷酸类似物耐药患者外周血白介素-21(IL-21)、γ-干扰素(γ-IFN)、IL-10及β-转化生长因子(β-TGF)的变化情况。方法以2012年7月—2015年7月所收治慢性乙型肝炎患者48例进行回顾性分析,根据患者是否出现核苷酸类似物耐药分为A组(无耐药,22例)与B组(耐药,26例)。另从同期健康检查的体检者中随机抽取23例作为对照组。测定3组的外周血IL-21、γ-IFN、IL-10及β-TGF水平并比较。结果 A、B组外周血IL-21水平低于对照组(P<0.0167),A组低于B组(P<0.0167);A、B组外周血IL-10、γ-IFN水平低于对照组(P<0.0167);A和B组拉米夫定应用比例差异有统计学意义(P<0.01)。结论核苷酸类似物的耐药会影响慢性乙型肝炎患者机体的免疫功能。 Objective To investigate changes of interleukin-21 (IL-21), γ-interferon (γ-IFN), IL-10 and β-transforming growth factor (β-TGF) of peripheral blood in chronic hepatitis B patients with drug resistance to nucleotide analogs. Methods Clinical data of 48 patients with chronic hepatitis B admitted between July 2012 and July 2015 was retrospectively analyzed, and the patients were divided into group A (no drug resistance, n=22) and group B (drug re-sistance, n=26) according to whether patients had drug resistance to nucleotide analogs or not, and then other 23 health-y examinees at the same period were selected randomly as control group. IL-21,γ-IFN, IL-10 and β-TGF levels of pe-ripheral blood were detected and compared among three groups. Results Compared with those in control group, IL-21 levels of peripheral blood in group A and B were lower (P〈0. 0167), and IL-21 level in group A was lower than that in group B (P〈0. 0167);IL-10 and γ-IFN levels of peripheral blood in group A and B were lower (P〈0. 0167). There was significant difference in Lamivudine medication between group A and B ( P〈0. 01 ) . Conclusion Drug resistance to nucleotide analogs can affect immune function of patients with chronic hepatitis B.
