摘要
例1男，28岁。躯干及四肢红斑伴瘙痒2个月，眉毛脱落1个月。查体见双侧眉毛全部脱落，背部散在分布黄豆至花生大红色斑疹，右大腿及双小腿伸侧见约十个直径1～4cm的圆形红色斑疹，界清，稍浸润，上均覆白色鳞屑，血CD4计数520个／ILL，CD4／CD8比值1．1。例2男，56岁。全身皮疹2个月。颈部及双侧腹股沟见数个无痛性肿大淋巴结，颜面、躯干及四肢密集分布黄豆至蚕豆大暗红色浸润性丘疹、斑块及结节，部分表面破溃并坏死，右小腿踝关节屈侧见3．0cm×2．0cm大溃疡，表面坏死和渗液，血CIM计数364个／μL；CD4／CD8比值O．15。例1血RPR1：128（＋），例2血RPR1：32（＋），例1和例2血TPPA、抗HIV抗体初筛及确诊试验皆阳性，均诊断为二期梅毒和HIV感染，予苄星青霉素治疗后皮疹已消退。
A 28-year-old male patient （ case 1 ） presents with pruritic erythema on his arms, legs and trunks for 2 months, accompanied by eyebrow loss for 1 month. Physical examination revealed approximately 10 red scaling macules, 1 to 4cm in diameter, on the trunk, right thigh and extensor surface of both shins. The slightly infiltrated lesion showed clear margins. CD4 T cell count was 520 cells/μL, CD4/CD8 ratio was 1.1. The serum rapid plasma reagin （RPR） test was positive （ titer 1 ： 128）. Case 2 was a 56-year-old male patient. The lesions affect the whole body for 2 months. On examination, his cervical and Inguinal lymph node appeared painless enlargement. His arms, legs, face and trunks appeared widely distribution of the soybean-sized dark red infiltrated papules, plaques and nodules. Necrosis and ulceration presented in part of the lesions. A large ulcer（ ulcerated area： 3.0cm×2.0cm） with necrotic exudates could be found in the flexor surface of the right ankle. CD4 T cell count was 364 cells/μL, CD4/CD8 ratio was 0.15. The serum rapid plasma reagin （RPR） test was positive （ titer 1 ： 32 ）. Serum TPPA, screening and confirmatory testing for HIV were all positive in two patients. The diagnose of secondary syphilis and HIV infection were made. All rashes disappeared after patients were treated by Benzathine Benzyl penicillin.
