摘要
目的 评价加巴喷丁对剖宫产术后患者静脉自控镇痛（PCIA）的影响。方法 98例ASAⅠ或Ⅱ级行剖宫产术并自愿接受静脉自控镇痛（PCIA）的孕产妇,按随机数字表法分为治疗组和对照组,每组49例。治疗组术前应用加巴喷丁,术后应用静脉自控镇痛;对照组仅术后应用静脉自控镇痛。比较两组产妇术后24 h的镇痛效果。结果 治疗组术后24 h各时点VAS评分低于对照组（P〈0.05）,术后0~4、4~8、8~12、12~24 h PCIA按压次数及PCIA药物总用量均少于对照组（P〈0.05）,治疗组术后镇静及恶心、呕吐发生率均低于对照组（P〈0.05）,治疗组术后镇痛整体优于对照组（P〈0.05）。新生儿出生后1 min、5 min、10 min Apgar评分均〉8分,15 min适应能力及神经行为评分（NACS评分）两组比较差异无统计学意义（P〉0.05）。结论 多模式镇痛中,术前口服加巴喷丁与对照组相比,可降低剖宫产术后疼痛,减少静脉自控镇痛药物量,提高产妇满意度。
Objective To evaluate the effect of gabapentin on patient-controlled intravenous analgesia （PCIA） after cesarean section. Methods Totally 98 cases of pregnant women with ASA Ⅰ or Ⅱ who received cesarean section and PCIA voluntarily were divided into treatment group and control group according to random number table,49 cases in each group. Treatment group was given gabapentin preoperatively and PCIA postoperatively;control group only received PCIA postoperatively. The 24 h postoperative analgesia effects were compared between the two groups. Results The VAS score at each time point within 24 h after operative in treatment group was less than control group （ P 〈 0. 05 ） ;the PCIA pressing numbers at 0 -4,4 -8,8 N 12 and 12 -24 h after operation and the total dosage of medicine at each time point in treatment group were all lower than control group （ P 〈 0. 05 ） ; the rate of sedation and nausea and vomiting in treatment group was all lower than control group （ P 〈 0. 05 ） ; the postoperative analgesia in treatment group was superior to control group （ P 〈 0. 05 ）. The 1 rain ,5 min and 10 rain Apgar scores of neonates were all more than 8 ; the difference in adaptability and Neurological and Adaptive Capacity Score （NACS） between the two groups was not significant （ P 〉 0.05 ）. Conclusion In multimodal analgesia, preoperative oral gabapentin can relieve the postoperative pain and reduce the dosage of medicine in PCIA, and improve the satisfaction of parturients.
