摘要
目的通过应用斑点追踪显像技术检测妊娠高血压综合征(PIH)对新生儿的右心室心肌节段的影响,进而评价妊高症对新生儿右心室功能的影响。方法采集60例PIH新生儿,包括妊娠高血压组(GH组)30例、子痫前期组(PE组)30例,以及60例正常新生儿(对照组)心脏的四腔心切面超声心动图,测量右心室各节段快速射血期峰值运动速度(Vs)和峰值应变率(SRs)、峰值应变(S)、快速充盈期峰值运动速度(Vd)和峰值应变率(SRd)。结果 120例新生儿中成功采集图像112例。右心室各组室间隔壁及游离壁Vs和Vd由基底段向心尖段逐渐减低,基底段与心尖段差异具有统计学意义(P<0.05),S、SRs和SRd在基底段、中间段和心尖段差异无统计学意义(P>0.05)。GH及PE组右心室游离壁各节段心肌Vs及Vd与对照组相比数值降低(P<0.05),GH及PE组右心室室间隔面各节段心肌Vd与对照组相比数值降低(P<0.05),GH及PE组右心室各节段心肌S、SRs和SRd与对照组相比大部分降低(P<0.05)。PE组右心室游离壁各节段心肌Vs及Vd与GH组相比数值降低(P<0.05)。结论斑点追踪显像技术可以检测新生儿右心室心肌节段功能的变化,PIH对新生儿右心室功能有影响,使部分心肌功能减低,PIH病情加重,新生儿部分心肌受损随之加重。
Objective Through the application of speckle tracking imaging technique for the examination of pregnant women with pregnancy - induced hypertension (PIH) on the impact of right ventrieular myocardial segments of the neonates and, in turn, to assess the PIH on impact of right ventricular function in neonata. Methods We collected echocardiography on four chamber of heart from 60 newborns, whose mothers were suffered from PIH, including 30 cases from gestational hypertension ( GH group) 30 cases from preeclampsia (PE group), and 60 cases of normal neonates (control group) and measured the peak velocity (Vs) at rapid ejection phase of each segment of right ventricle, peak strain rate (SRs) , peak strain (S) , peak velocity (Vd) at rapid fill- ing phase, and peak strain rate (SRd). Results Of those 120 neonates, 112 were successfully acquired the images. The Vs and Vd in each free wall and septal wall of the fight ventricle in each group were gradually reduced from the basal segments to the api- cal segments, with statistically significant difference between the basal segment and apical segments ( P 〈 0.05 ). The Vs and Vd in each myocardial segment of the right ventricle in GH and PE group were significantly decreased compared with those in control group ( P 〈 0.05 ). The Vd in each myocardial segment of the right ventricle in GH and PE group were significantly reduced com- pared with those in control group ( P 〈 0.05). The S, SRs and SRd in each myocardial segment of the right ventricle in GH and PE group were significantly decreased compared with those in control group ( P 〈 0.05 ). The Vs and Vd in each myocardial seg- ment of fi'ee wall of right ventricle in PE group was significantly decreased compared with that in GH group ( P 〈 0.05 ). Conclu- sion Speckle tracking imaging can detect changes in the function of fight ventricular myocardial segments in neonates.
