摘要
目的观察孟鲁司特联合布地奈德对咳嗽变异性哮喘患儿肺功能及免疫功能的影响。方法选择咳嗽变异性哮喘儿童60例,随机分为对照组和观察组,各30例。对照组常规吸入布地奈德气雾剂,观察组在常规吸入布地奈德气雾剂基础上加服孟鲁司特,2组连续用药3个月。观察2组治疗前后哮喘控制测试(C-ACT)评分、急性症状发作次数/月,包括日间喘息发作次数、夜间喘息发作次数以及肺功能指标。结果治疗2周后观察组C-ACT评分显著高于对照组;急性症状发作次数均较治疗前有明显改善(P<0.05),且治疗后观察组急性症状发作次数明显少于对照组(P<0.05);治疗3个月后肺功能指标均显著改善,且观察组各指标均显著优于对照组;治疗后2组Ig E、IL-4和TNF-α水平均显著降低(P<0.05),观察组Ig E、IL-4和TNF-α水平均低于对照组(P<0.05)。结论孟鲁司特联合布地奈德联合治疗儿童咳嗽变异性哮喘疗效显著,二者协同作用明显改善患儿的肺功能及免疫功能,值得推广应用。
Objective To observe the influence of montelukast combined with budesonide on pulmonary function and immunological function in infants with cough variant asthma （CVA）. Methods A total of 60 infants with CVA were selected and randomly divided into control group and observation group, with 30 cases for each group. Control group was treated with routine aerosol inha- lation of budesonide, on which basis observation group was added with aerosol inhalation of monte- lukast, for continuously 3 months. Childhood asthma control test （C-ACT） score before and after treatment, acute attack frequency （ times per month） including frequency of asthmatic attack at day- time and at night, and indexes of pulmonary function were observed in both groups. Results After 2 weeks treatment, observation group was evidently higher in C-ACT score than control group, and a- cute attack frequency was improved significantly than treatment before （P 〈 0.05 ）, which was nota- bly less in observation group than that in control group （ P 〈 0.05 ）. After 3 months treatment, the in- dexes of pulmonary function was prominently improved, which were apparently better in observation group than those in control group, and the levels of immunoglobulin E （IgE）, interleukin （IL）-4 and tumor necrosis factor-c~ （TNF-c~） decreased obviously （P 〈0.05 ） , which were prominently lower in observation group than those in control group （P 〈 0.05）. Conclusion Montelukast combined with budesonide has excellent efficacy in the treatment of infants with CVA, and has synergic action in im- proving infantile pulmonary function and immunological function, so it is deserving to be widely used in clinic.
