精神分裂症超高危人群的认知功能对照研究 被引量:12

摘要 目的探讨精神分裂症超高危人群的认知功能及相关因素。方法纳入38例精神分裂症超高危人群(UHR组)和38例健康人(对照组)作为受试者,所有受试者均进行阳性和阴性综合征量表(PANSS)评估精神病性症状,采用MATRICS公认认知成套测验(MCCB)评估认知功能,包括霍普金斯词语学习测验-修订版(HVLT-R)、简易视觉空间记忆测验-修订版(BVMT-R)、Stroop色词测验、持续操作测验(CPT)、连线测验(TMT)、精神分裂症简易认知评估测验(BACS-SCS)等,重点评估学习与记忆、注意能力/警觉性、信息处理速度等认知领域。结果与对照组相比,UHR组在霍普金斯词语学习测验中的HVLT-R2、Stroop色词测验中的词、色测验以及持续操作测验中的CPT3和CPT均分均差于对照组(P<0.05)。连线测验与年龄呈正相关(P<0.05),简易视觉空间记忆测验中的BVMT-R2、BVMT-R3、持续操作测验中的CPT1、CPT3、CPT均分均与年龄呈负相关(P<0.05),符号编码测验与受教育年限呈正相关(P<0.05)。结论精神分裂症超高危人群存在广泛的认知功能损害,认知功能的损害可能开始于青少年时期。 Objective To explore the cognitive function and related factors in ultra-high-risk psychosis population. Methods A total of 38 individuals with ultra-high-risk for psychosis( UHR group) and 38 healthy volunteers( control group) were selected and assessed with Positive and Negative Syndrome Scale( PANSS) for the psychotic symptoms and MATRICS Consensus Cognitive Battery( MCCB) for the cognitive function. Hopkins Verbal Learning Test-Revised( HVLT-R),Brief Visuospatial Memory Test-Revised( BVMT-R),Stroop Color Word Test,Continuous Performance Test( CPT),Trail Making Test( TMT),Symbol Coding Subtest of Brief Assessment of Cognition in Schizophrenia( BACS-SCS) were used to evaluate study and memory,attention / vigilance,information processing speed. Results The scores of HVLT-R2,words test and color test in Stroop Color Word Test,CPT3 and the average score of CPT in UHR group were significantly worse than those in control group( P〈0. 05).The TMT result was positively correlated with age in UHR group( P〈0. 05). The results of BVMT-R2,BVMT-R3,CPT1,CPT3 and the average score of CPT were negatively correlated with age in UHR group( P〈0. 05). BACS-SCS was positively correlated with education years in UHR group( P〈0. 05). Conclusion Widespread cognitive impairments exist among ultrahigh-risk psychosis population and the impairments may begin in their adolescence.
