Effects of comprehensive interventions on prevention and control of nosocomial infections

摘要 目的评价综合干预措施对医院感染预防控制效果的影响,为有效降低医院感染提供依据。方法选取医院2011-2015年住院患者为研究对象,调查患者133 482例,干预措施包括建立管理平台、推进医院感染预警信息系统、开展"医疗综合目标评估"、加强高风险科室医院感染管理等;对比干预前后医院感染率、Ⅰ类切口手术部位感染率、多药耐药菌医院感染率、手卫生依从率等指标的变化。结果采取干预措施后医院感染率明显降低,由1.80%降至1.06%,差异有统计学意义(P<0.01);普外科、骨科及其他外科Ⅰ类手术切口感染率均下降;全院Ⅰ类切口感染率从0.33%降至0.23%;多药耐药菌医院感染率明显下降,从0.48‰降至0.24‰;手卫生依从率呈逐年上升趋势,从38.2%升至87.2%。结论综合干预措施可有效降低医院感染。 OBJECTIVE To evaluate the effects of comprehensive interventions on prevention and control of nosocomial infections so as to effective reduce the incidence of the nosocomial infections.METHODS A total of 133 482 patients who were hospitalized from 2011 to 2015were recruited as the study objects.The interventions included establishing management platform,boosting the early warning information system of nosocomial infections,carrying out the comprehensive evaluation of health care,and strengthening the management of nosocomial infections in high-risk departments.The incidence of nosocomial infections,incidence of typeⅠincision infection,incidence of MDROs infections,and hand hygiene compliance rate were observed and compared before and after the intervention.RESULTS The incidence of nosocomial infections was remarkably reduced after the interventions were taken,dropping from 1.80%to 1.06%,and there was significant difference(P〈0.01).The incidence of typeⅠincision infections was decreased in the general surgery department,orthopedics department,and other surgery departments;the incidence of typeⅠincision infections dropped from 0.33%to 0.23%in the whole hospital.The incidence of MDROs infections was significantly decreased from 0.48‰to 0.24‰.The hand hygiene compliance rate was increased year by year,rising from 38.2%to 87.2%.CONCLUSIONThe comprehensive interventions can effectively reduce the incidence of the nosocomial infections.
