摘要
目的:观察血必净注射液对老年骨折并机械通气患者血液流变学的影响,探讨该类患者发生深静脉血栓(DVT)的机制。方法:60例患者随机分成对照组和治疗组,每组各30例。对照组行常规治疗,治疗组在对照组的基础上加用血必净注射液。测定2组患者治疗前后的血浆粘度、红细胞压积(HCT)、红细胞沉降率(ESR),测定2组患者治疗前后的氧合指数(Pa O2/Fi O2)和血乳酸值(Lac),统计机械通气时间、DVT发生率和28 d死亡率。结果:治疗组的ESR、全血还原粘稠度(比)、血浆粘稠度(比)、Lac均较对照组明显降低(P<0.05),DVT发生率较对照组低(P<0.05),且Pa O2/Fi O2水平较对照组升高更显著(P<0.05),机械通气时间较对照组明显缩短(P<0.05)。结论:血必净注射液可降低老年骨折并机械通气患者的血液粘稠和预防DVT的发生,同时可改善微循环和氧合,并缩短机械通气时间。
Objective： To observe the effects of Xuebijing injection on blood rheology in elderly fracture patients with mechanical ventilation, explore the mechanism of deep vein thrombosis （DVT） occurred in the patients. Methods： 60 patients were randomly divided into Control Group （30 cases） and Xuebijing Group （30 cases） . Xuebijing group received Xuebijing injection, besides physical therapy for prevention of deep venous thrombosis received by control group. Plasma viscosity, hematocrit （HCT）, erythrocyte sedimentation rate （ESR）, oxygenation index （OI） and blood lactic acid value （LAC）, mechanical ventilation time, incidence of DVT and 28-day mortality between two groups were compared. Results： In Xuebijing Group, ESR, whole blood reduced viscosity （ratio）, plasma viscosity （ratio）, Lac were significantly decreased lower than those in the control group （P 〈 0. 05 ） . The incidence of DVT was lower than the control group （P 〈 0. 05）, and the levels of OI were significantly increased than the control group （ P 〈 0. 05 ） . Mechanical ventilation time was significantly shortened （ P 〈 0.05 ） . Conclusion： Xuebijing injection can reduce blood viscosity, prevent the incidence of DVT, improve microcirculation and oxygenation, shorten the duration of mechanical ventilation in elderly fracture patients with mechanical ventilation.
