Etiological analysis on surveillance data of influenza in children in Hebei Province from 2009 to 2015

摘要 目的 分析2009-2015年河北省儿童流感样病例(influenza like illness,ILI)流感病原学特征,为河北省儿童流感的防控提供科学依据.方法 采集河北省流感监测哨点医院就诊的儿童ILI咽拭子标本,采用RT-PCR方法进行流感病毒核酸检测.结果 共采集咽拭子标本28 932例,检出流感病毒阳性标本5057例(阳性率为17.48%).按年度分析,6个年度流感病毒检出率依次为32.56%、9.53%、15.07%、12.29%、11.32%和18.81%,2009-2010年度显著高于其他年度(P<0.05).以B型流感病毒为优势型的年度有两个(2011-2012、2013-2014年度),其他年度为甲型H1N1和季节性H3N2流感交替流行或共同流行.年龄分布中,1岁以下年龄组检出率最低(7.74%),7-14岁年龄组最高(26.01%),检出率随年龄的增大而增高.季节分布显示,河北省流感为冬春季流行,呈单峰型,A型流行形成的峰时间靠前.结论 2009-2015年度河北省儿童中存在甲型H1N1、B型和季节性H3N2流感病毒的流行,不同年度有不同的流行优势型和流行特征. Objective To analyze the epidemiological characteristics of influenza in children during 2009-2015 in Hebei province,so as to provide scientific basis for its prevention and control.Methods The pharyngeal swabs specimens of influenza-like illness (ILI) cases in children were collected from influenza surveillance sentinel hospitals.Results 28 932 specimens were collected and detected by RT-PCR for influenza virus,in which 5057 specimens were positive,with positive rate being 17.48%.Among the six surveillance years,the positive rate of influenza virus was 29.18%,12.58%,12.97%,13.34%,11.64% and 15.95%,respectively,which peaked between 2009 and 2010 and was significantly higher than that in the other years (P 〈 0.05).The dominant pathogens in the two surveillance years (2011-2011,2013-2014) were influenza B virus,however,seasonal H3N2,influenza A (H1N1) pdm09 virus appeared alternately or common popular in the other years.The age distribution analysis showed that the infants who were younger than 1 years had the lowest infection rate(7.74%),which was significantly lower than that (26.01%) in the age group with age range 7-14 years.The detection rate was increased with patient' s age.During these years,there were six full single epidemic peaks,with popular in winter and spring.The peak formed by type A influenza virus was earlier than that of B influenza virus.Conclusions There exists prevalence of influenza virus during 2009-2015 in children in Hebei province including influenza A (H1N1)pdm09,influenza B and seasonal influenza A (H3N2) virus subtype,and the dominant type of influenza virus and epidemiological characteristics vary in different years.
