摘要
目的探讨MRI T2弛豫时间在腰椎间盘退行性变中的研究价值。方法前瞻性纳入2013年10月至2015年5月,因腰背部疼痛来我院就诊的患者50例(248个椎间盘),其中男24例,女26例,年龄18-78岁,平均(42±15)岁。行腰椎1.5 T MRI检查,常规矢状位T1WI及T2WI、横断位T2WI,并行正中矢状位T2 mapping成像,根据Pfirrmann标准将248个腰椎间盘分级,并测量髓核及纤维环前、后缘的T2值。采用单因素方差分析分别比较各级髓核(nucleus pulposus,NP)和纤维环(annulus fibrosus,AF)前、后缘T2弛豫时间差异有无统计学意义,并测量T2值与年龄、分级的Pearson相关性,分级与年龄的Pearson相关性。结果共计测量248个椎间盘,统计结果示:(1)腰椎间盘各分级NP的T2值随分级的增高而减小;(2)腰椎间盘各分级AF前缘T2值随分级的增高无明显变化;(3)腰椎间盘各分级AF后缘随T2值分级的增高呈下降趋势;(4)50例腰痛患者年龄与腰椎间盘分级呈弱正相关,年龄与椎间盘AF前、后缘T2值呈弱负相关,年龄与NP T2值呈中度负相关。腰椎间盘AF前、后缘T2值与各分级均呈弱负相关;腰椎间盘NP T2值与各分级呈高度负相关。结论腰椎间盘T2值可以很好地体现其退行性变的程度,并可以进行量化分析,T2 mapping成像技术可以为诊断腰椎间盘早期退行性变提供影像学依据。
Objective To assess the value of MRI T2 relaxation times to evaluate the lumbar intervertebral disc degeneration( IVDD). Methods Fifty patients( 24 males, 26 females, 248 discs) with the mean age of( 42 ± 15) years( range: 18- 78 years) with low back pain were prospectively enclosed between October 2013 and May 2015. All patients were examined by MRI at 1.5 T( sagittal T1 WI, T2 WI, axial T2 WI, multi-echo spin echo for T2 mapping). All discs were classified on the basis of Pfirrmann grading system into 5 stages of disc degeneration. T2 value of all nucleus pulposus( NP), anterior and posterior annulus fibrosus( AF) were measured and their variances in different grades were analyzed with one-way ANOVA test. Correlations between T2 value and age, grades, age and grades were analyzed with Pearson correlation test. Results( 1) T2 value of NP decreased while the IVDD grade increased;( 2) T2 value of anterior AF showed no obvious changes with the increasing of IVDD grade;( 3) T2 value of posterior AF decreased with the increasing of IVDD grade;( 4) Linear regression analysis between T2 value of NP and grades revealed significant negative verse correlation( P〈 0.01). Conclusions T2 value supports the feasibility of quantitatively assessing lumbar intervertebral disc degeneration. T2 mapping provides image reference for the early diagnosis of disc degeneration.
